Abstract
Objective To investigate the effect of anatomical reconstruction of medial patellofemoral ligament(MPFL)with double-bundle autologous anterior half of peroneus longus tendon(AHPLT)for treatment of recurrent patellar dis-location.Methods The 80 patients with recurrent patellar dislocation were treated by anatomical reconstruction of MPFL with double-bundle autologous AHPLT.The operative time and complications were recorded.The thickness of peroneus longus tendon on healthy side and affected side were measured using B-ultrasound.Lysholm score,Kujala score and IKDC score were used to evaluate the curative effect.Results The operation time was 45～60 min.All pa-tients were followed up for 18～36 months.All the incisions healed in one-stage after surgery.During the follow-up,the ankle joint function of the tendon cutting area recovered well,without pain and discomfort.The patellar extrapola-tion test and fear test at 20° kneeling knee were negative.After operation,3 patients had mild pain in the prepatellar region,and the other patients had no subjective discomfort in the knee joint.Six months after surgery,all patients com-pletely returned to their normal activities.There was no statistical difference in the thickness of peroneus longus tendon between the affected side and healthy side(P>0.05).At 6 months and the last follow-up,Lysholm score,Kujala score and IKDC score were higher than those before surgery(P<0.05).At the last follow-up,all patients had no re-currence of patellar dislocation or subluxation.Conclusions Autologous AHPLT is a selectable good graft for MPFL reconstruction.Anatomical reconstruction of MPFL with double-bundle autologous AHPLT is simple and minimally inva-sive,which can significantly improve knee joint function,meanwhile it has no significant effect on ankle joint function.关键词
