两种皮瓣修复末节指腹缺损的疗效比较

Abstract
Objective To compare the efficacy of digital artery retrograde island flap and free wrist transverse flap in the repair of distal finger pulp skin defect.Methods The 59 patients with finger pulp skin defects with exposed pha-langes were divided into group A(30 cases were treated with digital artery retrograde island flap)and group B(29 ca-ses were treated with free wrist transverse flap),according to different treatment methods.The operative time,VSS score,two-point discernment of skin flap,skin temperature of the affected finger tip,and the recovery grade of sensory function after peripheral nerve injury were recorded.The recovery of finger activity was evaluated by the evaluation standard of hand tendon repair of the Chinese Medical Association.Results All patients were followed up for 6 months.The operation time in group A was shorter than that in group B(P<0.01).Six months after surgery,VSS score,two-point discernment of skin flap,skin temperature of affected finger tip and sensory function recovery grade after peripheral nerve injury in group B were superior to those in group A(P<0.05).At 6 months after operation,there was no statistical difference in the recovery excellent-good rate of finger activity between the two groups(P>0.05).Conclusions Both the digital artery retrograde island flap and the free wrist transverse flap can achieve better efficacy in the treatment of distal finger abdominal defect,and the free wrist transverse flap has more advantages in skin appearance,skin temperature of distal finger abdomen,two-point discrimination and sensory nerve function re-covery.关键词
