经皮钢丝辅助复位结合加长型InterTan髓内钉固定治疗股骨转子下骨折

摘要
Abstract
Objective To investigate the curative effect of percutaneous wire-assisted reduction combined with extend-ed InterTan intramedullary nail fixation on femoral subtrochanteric fracture.Methods The 25 cases of femoral sub-trochanteric fractures with unsuccessful closed reduction were treated by percutaneous wire guided circumferential re-duction combined with extended InterTan intramedullary nail fixation.Fracture reduction,healing and complications were recorded.The Harris score was used to assess hip function.Results All patients were followed up for 12～24 months.After operation,the quality of fracture reduction was evaluated according to Baumgartner criteria:anatomical reduction or near-anatomical reduction in 24 cases,functional reduction in 1 case.The healing time of 24 patients was 4～7 months,and the healing time of 1 patient with diabetes and osteoporosis was delayed to 10 months after sur-gery.There were no postoperative complications such as infection,deep venous thrombosis of lower extremities,bone nonunion,and internal fixation failure.At 1 month postoperation,a case suffered from the ipsilateral femoral fracture at the distal intramedullary nail caused by a fall during double-crutch-assisted ambulation,and wires fracture occurred in a case at postoperative 12 months.At the last follow-up,the Harris score was used to evaluate hip function:excel-lent 17 cases,good6 cases,fair2 cases,with a excellent and good rate23/25.Conclusions The effect of percuta-neous wire-assisted reduction combined with extended InterTan intramedullary nail fixation in the treatment of femoral subtrochanteric fracture is satisfactory,which has the advantages of less trauma,simple operation,high quality of fracture reduction and fewer complications.关键词
