Effects of adenotonsillectomy and headgear-activator therapy on three-dimensional soft tissue contours in skele-tal Class Ⅱ high-angle patients

摘要

Abstract

Objective:To investigate the effects of adenotonsillectomy,headgear-activator orthopedic treatment,and their combination on three-dimensional soft tissue facial contours in skeletal Class Ⅱ high-angle patients.Methods:This ret-rospective study included 33 skeletal Class Ⅱ high-angle patients in their peak growth phase,categorized into three groups based on treatment:surgical group(9 cases,receiving adenotonsillectomy only due to ineligibility for orthopedic treatment),orthopedic group(15 cases,receiving headgear-activator treatment only due to ineligibility for surgery),and combined treat-ment group(9 cases,receiving both interventions).Facial three-dimensional soft tissue contours were captured and analyzed using the 3dMD imaging system with landmark-based measurements.Results:All three groups exhibited significant increases in facial contour dimensions and modest improvements in facial convexity.Surgical group:Increased ramus height and man-dibular body length;decreased mandibular plane angle and mandibular convexity angle.Orthopedic group:Increased anterior facial height,ramus height,mandibular body length,facial angle,and nasomental angle;decreased lower facial width,lower lip height/mandibular body length ratio,lower facial height/anterior facial height ratio,mandibular plane angle,and mandibular convexity angle.Combined group:Increased ramus height,mandibular body length,and nasomental angle;decreased mandibu-lar plane angle and mandibular convexity angle.Differences among interventions were subtle.Compared to the surgical group,the combined and orthopedic groups showed reductions in anterior lower facial height by 1.72 mm and 3.07 mum,respective-ly,though only the orthopedic group reached statistical significance(P=0.031).The lower facial height/anterior facial height ratio decreased by 0.027 mum and 0.032 mm in the combined and orthopedic groups,respectively,but neither was statistically significant.Conclusion:Adenotonsillectomy and headgear-activator orthopedic treatment promote mandibular growth by ad-dressing underlying etiological factors and correcting abnormal jaw morphology,respectively.The three-dimensional soft tissue contour changes induced by adenotonsillectomy,headgear-activator treatment,or their combination showed only minor varia-tions.Notably,headgear-activator treatment achieved more direct therapeutic effects,including improved vertical facial propor-tions.
