两种方法治疗踝关节骨折伴下胫腓联合韧带损伤的疗效比较
Comparison effect of two methods in the treatment of ankle fracture with distal tibiofibular syndesmotic injuries

Abstract
Objective To compare the efficacy of Endobutton loop plate and cannulated screw fixation in the treatment of ankle fracture with distal tibiofibular syndesmotic injuries.Methods The 90 patients of ankle fractures with distal tibiofibular syndesmotic injuries were divided into plate group(The 46 patients were treated with Endobutton loop plates)and screw group(The 44 patients were treated with cannulated screws)based on the different treatment meth-ods.The operative time for distal tibiofibular syndesmosis injuries fixation,partial weight-bearing ambulation time,fracture healing time,and postoperative complication incidence were compared between the two groups.The AOFAS ankle-hindfoot score was used to evaluate the recovery of ankle function.Results All patients were followed up for 8～24 months.There was no statistical difference in the operative time for distal tibiofibular syndesmosis injuries fixa-tion between the two groups(P>0.05).The partial weight-bearing ambulation time and fracture healing time were shorter in the plate group than those in the screw group(P<0.05).The incidence of postoperative complications was higher in the screw group than the plate group(P<0.05).At the final follow-up,there was no statistical difference in the excellent and good rate of the AOFAS ankle-hindfoot score between the two groups(P>0.05).Conclusions Both Endobutton loop plate and cannulated screw fixation are effective methods for treating ankle fractures with distal tibiofibular syndesmotic injuries.Compared with cannulated screw fixation,Endobutton loop plate fixation has the ad-vantages of convenient operation,elastic fixation,and early weight-bearing for treatment of lower tibiofibular ligament injuries.It is more conducive to ankle joint function recovery and reducing the occurrence of postoperative complica-tions,and which can avoid the trauma and related costs of secondary surgery for screw removal.关键词
