血压变异性对急性缺血性脑卒中取栓术患者预后的影响
Impact of blood pressure variability on the prognosis of patients undergoing mechanical thrombectomy for acute ischemic stroke

Abstract
Objective To explore the predictive value of blood pressure variability(BPV)for the prognosis after thrombectomy in patients with acute ischemic stroke(AIS).Methods A total of 173 patients with acute ischemic stroke(AIS)who underwent mechanical thrombectomy at the First People's Hospital of Lianyungang from January 2021 to December 2022 were retrospectively included.The blood pressure data within 7 days after the operation were collected,and the coefficient of variation(CV)of blood pressure and the slope(m)of dynamic changes were calculated.The association between BPV and prognosis through multivariate logistic regression analysis was evaluated.Results Univariate analysis showed that BPV,age,scores on admission,etc.were associated with the prognosis(all P＜0.05).Multivariate regression analysis indicated that m was an independent predictor of poor prognosis(OR=1.03 8,95％CI=1.013-1.054),suggesting that for every 1％increase in m,the risk of poor prognosis increased by 3.8％.The receiver operating characteristic(ROC)curve showed that the area under curve(AUC)of m was 0.824.When the cut-off value was 8.61,the sensitivity was 0.712 and the specificity was 0.912.Conclusions BPV is an effective predictor of poor prognosis after thrombectomy in patients with acute ischemic stroke.Monitoring and controlling postoperative blood pressure fluctuations,particularly BPV,can improve patient outcomes and provide robust clinical management guidance.关键词
