超声造影联合染料法在乳腺癌前哨淋巴结活检中的应用
Application of Contrast-Enhanced Ultrasonography Combined with Dye Tracing Method in Sentinel Lymph Node Biopsy for Breast Cancer

Abstract
Objective:To evaluate the feasibility of contrast-enhanced ultrasound combined with dye tracing method for sentinel lymph node(SLN)biopsy(SLNB)in patients with breast cancer.Methods:69 patients with early breast cancer who were treated in our hospital from January 2022 to June 2024 were collected.Contrast-enhanced ultrasound with carbon nanoparticles suspension injection and methylene blue were used for SLNB.The detection rate and number of SLN identified by contrast-enhanced ultrasound,methylene blue a-lone,and the intraoperative combined method(contrast-en-hanced ultrasound and dye)were recorded respectively.Fol-lowing SLNB,axillary lymph node dissection was performed to determine lymph node status,to assess the false-negative rate,sensitivity,and diagnostic accuracy of the intraoperative combined method for SLNB.Results:The SLN detection rate of the intraoperative combined method was significantly higher than that of the contrast-enhanced ultrasound method(P=0.005)and methylene blue(P=0.028)method.The number of SLN detected by the combined method was significantly high-er than that by the contrast-enhanced ultrasound method and the methylene blue method(P＜0.01,respectively).Among 22 cases with axillary lymph node metastasis,SLNB yielded false-negative results in 2 cases.The false-negative rate was 9.1％(2/22),sensitivity was 90.9％(20/22),and the accuracy of axillary lymph node prediction was 97％(65/67).The num-ber of positive SLN detected by the intraoperative combined method correlated with the pathological results of axillary lymph node dissection(r=0.668,P＜0.001).Conclusion:Contrast-enhanced ultrasound combined with dye tracing method for SLNB provides real-time lymphatic mapping and accurate SLN localization.This approach achieves high SLN detection rates and low false-negative rates,offering a clinically feasible alternative for SLNB in Tibetan regional hospitals lacking nuclear medicine facilities.关键词
