Abstract
Objective:To explore the influence of community chronic disease management model on blood pressure and quality of life of hypertensive patients.Methods:A total of 110 hypertensive patients were selected from Wangquan Community Health Service Center in Shunyi District,Beijing from April 2023 to April 2024.Patients were divided in-to groups according to the management mode,and 55 patients in the control group were included in the routine man-agement.The observation group included 55 cases,and implemented community chronic disease management based on the control group.The intervention effects of the two groups were evaluated from two aspects,blood pressure and quality of life.Results:After the intervention,compared with the control group,the blood pressure(diastolic blood pressure and systolic blood pressure)in the observation group was lower,and the SF-36 score was higher,P<0.05.Conclusion:The management mode of chronic diseases in community is effective in improving blood pressure and quali-ty of life of hypertensive patients.关键词
