摘要

Abstract

Objective:To investigate the trends of non-invasive hemodynamic parame-ters during pregnancy and their impact on fetal birth weight.Methods:A retrospective analysis was conducted on the non-invasive hemodynamic test results of 1 839 pregnant women who un-derwent prenatal checkups at hospital from June 2024 to January 2025.The trends of non-inva-sive hemodynamic parameters were analyzed for 713 women in the first trimester(8~13 weeks),481 women in the second trimester(14~27 weeks),and 645 women in the third tri-mester(28~36 weeks).Maternal and fetal data from 648 pregnant women with pregnancy out-comes were collected to preliminarily explore the impact of non-invasive hemodynamic parame-ters in the first,second,and third trimesters on fetal birth weight.Results:During the first,sec-ond,and third trimesters,there was no significant change in stroke volume(SV)among pregnant women(P>0.05).However,significant changes were observed in heart rate(HR),cardiac out-put(CO),systemic vascular resistance index(SVRi),systemic vascular resistance(SVR),and mean brachial artery pressure(MBAP)(all P<0.05).HR and CO showed a positive correlation with gestational age,while SVR and SV exhibited a negative correlation(all P<0.05).Among the 648 pregnant women with pregnancy outcomes,551(85.0%)delivered appropriate-for-ges-tational-age(AGA)infants,43(6.6%)delivered large-for-gestational-age(LGA)infants,and 54(8.3%)delivered small-for-gestational-age(SGA)infants.A comparison among the three groups revealed that cardiac timing index(CTI)showed a consistent trend from early to late pregnancy:the CTI of women who delivered LGA infants was significantly lower than that of the AGA and SGA groups.Multivariable logistic regression analysis yielded the following significant findings:Mid-trimester CTI demonstrated a significant inverse association with fetal birth weight categories.Specifically,each unit increase in CTI was associated with a 1.4%reduction in the likelihood of advancing to a higher birth weight category(e.g.,transitioning from AGA to LGA or from SGA to AGA)(OR=0.986,95%CI:0.976~0.996).Third-trimester HR showed a positive correlation with birth weight classification progression.Each 1-beat-per-minute increase in maternal HR was associated with a 5%greater probability of the newborn being classified in-to a higher weight category(e.g.,progression from AGA to LGA)(OR=1.05,95%CI:1.006~1.096).Conclusion:Non-invasive hemodynamic parameters undergo significant changes as pregnancy progresses.The non-invasive hemodynamic indices CTI and HR are significantly as-sociated with fetal growth and development.
