超声影像组学在青年卵圆孔未闭诊断中的应用研究
Application of Ultrasound Radiomics in Diagnosis of Patent Foramen Ovale in Young Patients

Abstract
Objective To investigate the application of ultrasound radiomics in the diagnosis of patent foramen ovale(PFO)in young patients.Methods 80 young patients with cryptogenic stroke in Pingxiang People's Hospital,Jiangxi Province from January 2022 to June 2023 were selected as study objects.Transthoracic echocardiography(TTE)foaming test confirmed 40 positive and 40 negative patients as the modeling set.Support vector machine(SVM)classifier was used to establish the diagnosis model of PFO to realize the function of screening young PFO with conventional apical four-chamber echocardiographic image.And the receiver operator characteristic(ROC)curve was used to evaluate the differentiation and predictive efficiency of the model.Another 60 cases of young patients with cryptogenic stroke from June 2023 to December 2023 were selected as validation set,and the above model screening diagnosis and TTE foam test were respectively used to detect and compare the diagnostic efficiency of the two groups of technical methods.Results Univariate and multivariate analysis results showed that gender,age,history of hypertension,history of hyperhomocysteinemia,and increase of atrial septal motion amplitude were the influencing factors of PFO in young patients with cryptogenic stroke between the two groups(all P<0.05).TThe prediction model constructed by SVM showed that the order of importance of influencing factors was age>hyperhomocysteinemia>hypertension>gender>increase of atrial septal motion amplitude.Internally verified ROC results showed that the area under the curve(AUC)was 0.871(95%CI:0.781~0.960),sensitivity was 0.867,and specificity was 0.767.The external verified ROC results showed that AUC was 0.818(95%CI:0.709~0.928),sensitivity was 0.767,specificity was 0.800,and the prediction model had better consistency and higher prediction value.The detection rate of SVM model was higher than that of foam experiment,with statistically significant difference(P<0.05).Conclusions Utrasound radiomics combined with SVM classifier has higher application value in the diagnosis of PFO in young patients,with high diagnosis accuracy and convenient operation,which is conducive to clinical promotion and application.关键词
