多普勒超声检查在腹膜透析超滤衰竭动静脉内瘘术前评估中的应用价值
The Application Value of Doppler Ultrasound Examination in Preoperative E-valuation of Arteriovenous Fistula in Peritoneal Dialysis Ultrafiltration Failure

Abstract
Objective To explore the application value of Doppler ultrasound examination in preoperative evaluation of arteriovenous fistula in peritoneal dialysis ultrafiltration failure.Methods This study included 80 patients admitted to the Nephrology Department of our hospital from January 2021 to December 2024 who underwent arteriovenous fistula surgery due to peritoneal dialysis ultrafiltration failure.All patients underwent Doppler ultrasound examination and completed arteriovenous fistula surgery.According to the surgical results three months later,the patients were divided into a successful group and a failed group.The radial artery diameter,peak systolic velocity(PSV),end diastolic velocity(EDV),mean velocity(Vmean),blood flow,and intima-media thickness(IMT)of the vessel wall were compared between two groups.ROC curve was drawn to analyze the evaluation value of various ultrasound indicators for the success of arteriovenous fistula surgery.Results Among the 80 enrolled patients,58 patients had successful arteriovenous fistula surgery,with a success rate of 72.5％.No significant difference showed in general information such as gender ratio,age,disease duration,and etiology between the successful and failed groups(P＞0.05).No significant difference was found in the IMT of the radial artery between the two groups of patients(P＞0.05).However,the internal diameter,PSV,EDV,Vmean and blood flow volume of the successful group were significantly higher than those of the failure group(P＜0.05).ROC curve analysis results showed that the internal diameter,PSV,EDV,Vmean and blood flow volume all had significant evaluation values for the success of arteriovenous fistula surgery(AUC＞0.7,P＜0.05).Conclusions Doppler ultrasound examination can effectively evaluate the preoperative vascular status of arteriovenous fistula in patients with peritoneal dialysis ultrafiltration failure,provide objective ultrasound reference information for surgical results.关键词
