神经内镜辅助下脑内血肿清除术与显微镜下脑内血肿清除术治疗高血压性基底核区脑出血的临床效果对比
Comparison on Clinical Effect of Neuroendoscopy-Assisted Intracerebral Hematoma Removal and Microscopic Intracerebral Hematoma Removal in the Treatment of Hypertensive Basal Ganglia Hemorrhage

Abstract
Objective To compare and analyze the clinical effect of neuroendoscopy-assisted intracerebral hematoma removal and microscopic intracerebral hematoma removal in the treatment of hypertensive basal ganglia hemorrhage(HBGH).Methods 76 patients with HBGH admitted to our hospital from January 2021 to September 2024 were selected and divided into two groups according to the random number table method.38 patients in group A underwent neuroendoscopy-assisted intracerebral hematoma removal,and 38 patients in group B underwent microscopic intracerebral hematoma removal.The perioperative indicators,neurological function(NIHSS,GCS,mRS)scores and brain injury biomarkers(GFAP,S-100β)levels before and 3 months after operation were compared between the two groups,and the postoperative complications of the two groups were counted.Results The hematoma clearance rate at 24 h after operation in group A was higher than that in group B,the intraoperative blood loss was less than that in group B,and the incision length,operation time and hospitalization time were shorter than those in group B(P＜0.05).At 3 months after operation,the NIHSS score and mRS score of group A were lower than those of group B,and the GCS score was higher than that of group B(P＜0.05).The levels of serum GFAP and S-100β in group A were lower than those in group B at 3 months after operation(P＜0.05).No significant difference was found in the total incidence of postoperative complications(2.63％vs.15.79％)between the two groups(P＞0.05).Conclusions The safety of neuroendoscopy-assisted intracerebral hematoma removal and microscopic intracerebral hematoma removal in the treatment of HBGH is comparable,however,compared with microscopic intracerebral hematoma removal,neuroendoscopy-assisted intracerebral hematoma removal is more helpful to optimize perioperative indicators and improve the levels of brain injury biomarkers,neurological function,state of consciousness and prognosis of patients with HBGH.关键词
