Impact of Central-Peripheral Theory-Based Swallowing Therapy Device Combined with Repetitive Transcranial Magnetic Stimulation on Swallowing Function,Electromyographic Signals,and Morphology of Patients with Post-Stroke Dysphagia

Abstract
Objective To explore the clinical effect of central-peripheral theory-based swallowing therapy device combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of patients with post-stroke dysphagia(PSD).Methods 80 PSD inpatients admitted to our hospital from April 2024 to February 2025 were selected and divided into control group(n=40)and study group(n=40)by a random number table method.Both groups were given symptomatic drug treatment and routine swallowing rehabilitation,while the control group was treated with swallowing therapy device,and the study group was treated with rTMS on the basis of the control group for a total of 4 weeks.The Kubota Water Swallowing Test,Standard Swallowing Function Assessment Scale(SSA),Swallowing Disorder Specific Quality of Life Scale(SWAL-QOL),and National Institutes of Health Stroke Scale(NIHSS),sEMG amplitude of suprahyoid and lingual muscles,thickness of suprahyoid muscles before and after intervention,and adverse reaction were compared between the two groups.Results Before intervention,no statistical difference was found in the Kubota Water Swallowing Test,the scores of SSA,SWAL-QOL and NIHSS,the sEMG amplitude of suprahyoid and lingual muscles,thickness of suprahyoid muscles between the two groups(P>0.05).After 4 weeks of intervention,the Kubota Water Swallowing Test and SSA,NIHSS scores of the study group were significantly lower than those of the control group,and the SWAL-QOL score was significantly higher than that of the control group(P<0.05).After 4 weeks of intervention,the sEMG amplitude of suprahyoid and lingual muscles,and the thickness of suprahyoid muscles in the study group were significantly higher than those in the control group(P<0.05).The incidences of aspiration pneumonia and nasal feeding dependence in the study group were significantly lower than those in the control group(P<0.05).Conclusions Central-peripheral theory-based swallowing therapy device combined with rTMS can significantly enhance the electromyographic signals and thickness of the swallowing core muscles,improve the swallowing function,and reduce the adverse reactions in PSD patients,which is worthy of clinical promotion and application.关键词
