增强磁共振神经成像结合肌电图在ICU-AW患者臂丛神经损伤评估中的价值
Value of Enhanced Magnetic Resonance Neurography Combined with Elec-tromyography in the Evaluation of Brachial Plexus Nerve Injury in Patients with ICU-AW

Abstract
Objective To investigate the value of enhanced magnetic resonance neurography(eMRN)combined with electromyography(EMG)in the evaluation of brachial plexus nerve injury in patients with intensive care unit-acquired weakness(ICU-AW).Methods A total of 60 ICU-AW patients admitted to our hospital from December 2021 to December 2024 were selected and underwent eMRN,EMG examinations in our hospital.The value of eMRN,EMG,and the combination of the two in the evaluation of brachial plexus nerve injury in ICU-AW patients was analyzed.Results Among the 60 patients,50 cases were clinically diagnosed with brachial plexus nerve injury.Taking clinical diagnostic results as the standard,the sensitivity and specificity of eMRN in evaluating brachial plexus nerve injury in ICU-AW patients were 88%and 60%,respectively;the sensitivity and specificity of EMG in evaluating brachial plexus nerve injury in ICU-AW patients were 82%and 70%,respectively;the sensitivity and specificity of eMRN combined with EMG in evaluating brachial plexus nerve injury in ICU-AW patients were 96%and 80%,respectively.The consistency between eMRN in evaluating brachial plexus nerve injury in ICU-AW patients and the clinical diagnosis results was general(Kappa=0.444);the consistency between EMG in evaluating brachial plexus nerve injury in ICU-AW patients and the clinical diagnosis results was general(Kappa=0.419);the consistency between eMRN combined with EMG in evaluating brachial plexus nerve injury in ICU-AW patients and the clinical diagnosis results was good(Kappa=0.760).The combined Kappa value was higher than that of eMRN and EMG alone(P<0.05).The coincidence rate of the two combined detection for brachial plexus nerve injury in ICU-AW patients was 93.33%,higher than that of eMRN and EMG alone;the missed detection rate and misdiagnosis rate were lower than those of eMRN and EMG alone.Conclusions The combined detection of eMRN and EMG is significantly better than single method in the evaluation of brachial plexus nerve injury in ICU-AW patients.eMRN is not sensitive to early nerve injury and is susceptible to artifacts.EMG-dependent muscle electrical activity may be missed or misdiagnosed.The combined application provides a new basis for the accurate evaluation of brachial plexus nerve injury in ICU-AW patients through"morphological-functional"complementarity,highlighting the clinical value of multimodal technology combination.关键词
