超声造影联合超声弹性成像在校正C-TIRADS甲状腺结节分类中的应用价值
Application Value of Contrast-Enhanced Ultrasound Combined with Ultra-sound Elastography in Correcting C-TIRADS Classification of Thyroid Nod-ules

Abstract
Objective To explore the application value of contrast-enhanced ultrasound(CEUS)combined with ultrasound elastography(UE)in correcting the Chinese Thyroid Imaging Reporting and Data System(C-TIRADS)classification of thyroid nodules.Methods The clinical data and imaging data of 650 patients with thyroid nodules who underwent thyroid conventional ultrasound,CEUS and UE examination and had clear pathological results in Luoyang Central Hospital from January 2020 to March 2024 were retrospectively collected.The pathological examination results showed 342 benign nodules and 335 malignant nodules.The results of pathological examination were used as the"gold standard",the C-TIRADS grading was performed by conventional ultrasound,and the C-TIRADS grading was corrected by CEUS combined with UE examination.The diagnostic efficacy of conventional ultrasound,CEUS and UE alone and in combination for benign and malignant thyroid nodules was compared.Results The sensitivity,specificity,positive predictive value and negative predictive value of conventional ultrasound in the diagnosis of thyroid nodules were 93.13%,74.56%,78.19%and 91.72%,respectively;CEUS combined with UE was 91.04%,84.21%,85.36%and 93.50%,respectively;the area under the ROC curve was 0.868 and 0.932 respectively,with statistically significant difference(P<0.05).Conclusions CEUS combined with UE has higher diagnostic accuracy than conventional ultrasound in the classification of thyroid nodules,especially in improving the diagnostic accuracy of C-TIRADS 4a and 4b nodules,and can reduce missed diagnosis and misdiagnosis,enhance the diagnostic efficacy of benign and malignant thyroid nodules.关键词
