小半夏加茯苓汤调控PI3K/Akt通路降低大鼠炎症程度防治化疗性呕吐的机制研究
Mechanism Study of Xiaobanxia and Fuling Decoction in Preventing and Treating Chemotherapy-Induced Nausea and Vomiting by Regulating the PI3K/Akt Pathway to Reduce Inflammation in Rats

Abstract
Objective:To investigate the mechanism by which Xiaobanxia and Fuling Decoction regulates the PI3K/Akt pathway to reduce inflammation caused by cisplatin in rats and prevent chemotherapy-induced nausea and vomiting(CINV).Methods:A CINV rat model was established through kaolin feeding and cisplatin injection.The rats were randomly divided into two batches(24 h and 72 h observation groups),with each batch further divid-ed into six groups:blank control group,model group(cisplatin 6 mg/kg),positive control group(ondansetron),and low-,medium-,and high-dose Xiaobanxia and Fuling Decoction groups(equivalent to crude herb doses of 1.1,2.2,and 4.4 g/mL,respectively).Kaolin intake,food consumption,body weight changes,gastric residue volume,stomach weight,and carbon propulsion length in the small intestine were recorded.Levels of IL-6,TNF-α,and EGFR in serum were measured.Hematoxylin and eosin(H&E)staining was performed on gastric and ileal tissues to observe inflammatory pathological changes.Western blotting and immunohistochemistry were used to detect the protein expression levels of PIK3CA,Akt1,and NF-κB in the stomach tissue.Results:Compared with the blank control group,the model group showed significantly increased kaolin intake,24 h gastric residue volume,expres-sion levels of PIK3CA,Akt1,and NF-κB proteins,as well as elevated levels of IL-6,TNF-α,and EGFR inflamma-tory factors(P<0.01).Histopathological analysis revealed significant structural damage and inflammatory infiltra-tion in gastric and ileal tissues.Compared with the model group,the medium-and low-dose Xiaobanxia and Ful-ing Decoction groups showed reduced kaolin intake,increased body weight and food intake over 24 h,and de-creased expression levels of PIK3CA,Akt1,NF-κB proteins,along with reduced levels of IL-6,TNF-α,and EGFR(P<0.01).Conclusion:The medium-and low-dose groups of Xiaobanxia and Fuling Decoction can inhibit the PI3K-Akt signaling pathway,reduce cisplatin-induced inflammation in rats,alleviate gastrointestinal inflammatory damage,relieve vomiting,and improve symptoms such as anorexia and weight loss.关键词
