醒脑开窍针刺法对急性缺血性卒中铁代谢影响及疗效随机对照研究
Effectiveness of Xingnao Kaiqiao Acupuncture on Iron Metabolism and Efficacy in Acute Ischemic Stroke:A Randomized Controlled Trial

Abstract
Objective To observe the effects of Xingnao Kaiqiao acupuncture on iron metabolism and clinical efficacy in patients with acute ischemic stroke.Methods A total of 167 patients with acute ischemic stroke were screened,80 patients were included and randomly assigned to an acupuncture group and a sham acupuncture group,40 patients each.The acupuncture group was treated with the Xingnao Kaiqiao acupuncture method,selecting bilateral Neiguan(PC6),Shuigou(DU26),bilateral Sanyinjiao(SP6),and Baihui(DU20)acupoints.And the sham acupuncture group was treated with the non-acupuncture and non-penetrating method for 3 weeks.To evaluate the clinical efficacy,the Modified Barthel Index(MBI),National Institutes of Health Stroke Scale(NIHSS),Fugl-Meyer Assessment Scale(FMAS),Functional Independence Measure(FIM),Berg Balance Scale(BBS),Mini-Mental State Examination(MMSE)and Pittsburgh Sleep Quality Index(PSQI)Scale of the patients were assessed before and after the treatment.The levels of serum iron metabolism were observed,including serum iron(SI),serum ferritin(SF),transferrin(TF),total iron binding capacity(TIBC),and transferrin saturation(TS).Additionally,the levels of various serological indicators were measured,including interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),myeloperoxidase(MPO),reactive oxygen species(ROS),glutathione(GSH),and malondialdehyde(MDA).The blood flow velocity of major intracranial vessels was also recorded using transcranial Doppler ultrasound.The patients that complied study protocol,completed all follow-ups were included in the per protocol set(PPS).The patients who received at least one treatment after randomization and had safety evaluation data were included in the safety set(SS).Results Compared with the sham acupuncture group,the BMI score,FMAS score and FIM score in the acupuncture group increased(P＜0.01,P＜0.05),the TF,TIBC,ROS and GSH increased(P＜0.05),and the blood flow rate of anterior cerebral artery(ACA),middle cerebral artery(MCA)and basilar artery(BA)increased(P＜0.01,P＜0.05).PSQI score,SF,IL-6,TNF-α,MPO,and MDA decreased(P＜0.01,P＜0.05).Conclusion Xingnao Kaiqiao acupuncture can effectively improve limb mobility,balance serum iron metabolism,and improve living ability in patients with acute ischemic stroke.(Chinese Clinical Trial Registry No.ChiCTR2300073296).关键词
