Efficacy comparison of growth factor therapies for diabetic foot ulcers:A systematic review and network Meta-analysis

Abstract
Objective:To systematically evaluate and compare the efficacy of various growth factors in treating diabetic foot ulcers,and provide a basis for clinical treatment choices.Methods:A systematic literature review was performed,encompassing 24 randomized controlled trials involving 1,619 patients with diabetic foot ulcers.A network meta-analysis was conducted to assess the comparative efficacy of epidermal growth factor(EGF),platelet-rich plasma(PRP),and platelet-derived growth factor(PDGF),fibroblast growth factor,(FGF),vascular endothelial growth factor,(VEGF)and traditional treatment for diabetic foot ulcers.Data extracted included basic characteristics,intervention details,and research results,etc.The Cochrane risk of bias tool was utilized to evaluate the methodological quality of the included studies,and the included clinical studies were statistically analyzed using Stata 14.2 software and the Network and Mvmeta packages.Results:Among the available treatment modalities,EGF was significantly more effective than VEGF in improving the healing rate of ulcer wounds(OR 2.46,95%CI 0.4-4.52),and PRP treatment was significantly better than conventional treatment(OR 1.19,95%CI 0.84-1.55)and FGF treatment(OR 2.40,95%CI 0.37-4.42),and the difference was statistically significant(P<0.05).The difference between any two treatment modalities of EGF,PRP,conventional treatment and FGF was not statistically significant in terms of the healing time of the ulcer wounds(P>0.05).The difference between any two treatment modalities of PRP,EGF,FGF,VEGF,and PDGF,compared with conventional treatment,was not statistically significant for the ulcer wounds(P>0.05).PRP treatment modality significantly reduced the length of hospitalization of the patients compared to the conventional treatment[mean(MD)-1.65,95%CI-3.17--0.12;P<0.05].Ranking the treatment regimens according to SUCRA results,EGF treatment modality was the most advantageous in terms of increased wound healing rate,shorter wound healing time,shorter hospitalization time,and fewer adverse events,followed by PRP treatment regimen.Conclusions:EGF comprehensively shows significant advantages in improving the healing rate,healing speed and shortening the length of hospital stay of diabetic foot ulcers,which is a new treatment modality with good prospects.关键词
