基于网络药理学探讨黄芩清热除痹胶囊改善类风湿关节炎患者免疫炎症反应的作用机制
The effect mechanism of Huangqin Qingre Chubi Jiaonang(黄芩清热除痹胶囊)in improving immune inflammatory response in patients with rheumatoid arthritis

Abstract
Objective To investigate the effect mechanism of Huangqin Qingre Chubi Jiaonang(黄芩清热除痹胶囊)in improving immune-inflammatory responses in rheumatoid arthritis(RA)patients.Methods Clinical data from 972 RA patients in the rheumatology department of the first affiliated hospital of Anhui university of Chinese medicine were analyzed.Data mining techniques were employed to evaluate the clinical efficacy of Huang-qin Qingre Chubi Jiaonang in modulating immune-inflammatory indicators,with further validation through a random walk model.Network pharmacology was utilized to predict potential therapeutic targets,pathways,and active compo-nents,followed by molecular docking to verify binding activities between core targets and active ingredients.En-zyme-linked immunosorbent assay(ELISA)was conducted to detect serum level changes of core targets pre-and post-treatment.Results Post-treatment analysis revealed significant reductions in rheumatoid factor(RF),hyper-sensitive C-reactive protein(Hs-CRP),erythrocyte sedimentation rate(ESR),immunoglobulin(Ig)A,IgG,comple-ment©3,and C4 levels(P<0.05).The random walk model demonstrated sustained correlations between Huang-qin Qingre Chubi Jiaonang intervention and improvements in RA-related biomarkers(RF,Hs-CRP,ESR,C3,C4,IgA,IgG).Network pharmacology identified 7 core therapeutic targets.Geneontology(GO)enrichment analysis in-dicated predominant involvement in biological processes including response to lipopolysaccharide,response to inor-ganic substance,response to wounding,and apoptotic signaling pathway.Kyoto encyclopedia of genes and genomes(KEGG)pathway analysis revealed 293 signaling pathways,notably PI3K-Akt,NF-κB,apoptosis,estrogen,and VEGF pathways.Molecular docking demonstrated strong binding affinities(lowest binding energy)between active components(kaempferol,baicalein,quercetin,acacetin)and core targets(TP53,AKT1,JUN).ELISA results showed significantly decreased serum AKT1 and JUN levels,with elevated TP53 levels after treatment(P<0.01).Conclusion Huangqin Qingre Chubi Jiaonang ameliorates RA immune-inflammatory responses by targeting AKT1,TP53,and JUN to regulate PI3K-Akt,NF-κB,and VEGF signaling pathways.关键词
