老年慢性病共病患者认知功能差异的分解研究
A decomposition study of disparities in cognitive function among the eld-erly with chronic comorbidities

Abstract
Aim:To analyze the influencing factors of cognitive function and their contributions to cognitive function in the elderly with chronic comorbidities.Methods:Using data from the 2020 China Health and Retirement Longitudinal Study(CHARLS),5 902 elderly people(≥60 years)with chronic comorbidities were selected as study subjects.Then their cog-nitive function was assessed.The OLS regression model was used to analyze the factors influencing cognitive function of the study population.The Shapley value decomposition method was used to analyze the contribution of influencing factors to their cognitive function.The Oaxaca-Blinder decomposition method was used to analyze gender differences in cognitive function in study population.Results:The cognitive function score of the elderly with chronic comorbidities was(10.15±4.19).The results of OLS regression analysis showed that age,gender,nightly sleep duration,physical activity,drinking status,amount of children,marital status,education degree,place of residence,annual personal income,and basic health insurance were the factors influencing cognitive function in the elderly with chronic comorbidities[β(95%CI)was-0.346(-0.552--0.140),-1.042(-1.271--0.813),-1.255(-1.807--0.703),-1.234(-1.552--0.916),-0.329(-0.543--0.116),-0.568(-0.804--0.332),0.514(0.264-0.765),2.569(2.360-2.778),1.059(0.822-1.296),1.191(0.961-1.421),0.593(0.098-1.088)].The results of Shapley value decomposition method showed that the social structural factors(education degree,annual personal income,place of residence)was the most important factor.The results of Oaxaca-Blinder decomposition method showed that 39.71% of the difference in cognitive function among the elderly with chronic comorbidities between different genders was related to individual characteristics,including education de-gree,drinking status,annual personal income,smoking status and physical activity,and 60.29% to gender differences.Con-clusion:The cognitive function of the elderly with chronic comorbidities in China is not optimistic,and relevant policy meas-ures should be taken for early intervention.Special attention should be paid to the elderly with chronic comorbidities who have low education degree,low income,and live in rural areas.关键词
