基于随机行走模型评价健脾清热通络方对强直性脊柱炎湿热痹阻证患者免疫炎症指标的影响
Evaluation of the effect of Jianpi Qingre Tongluo Fang(健脾清热通络方)on immune-inflammatory indicators in patients with ankylosing spondylitis and dampness-heat obstruction syndrome based on a random walk model

Abstract
Objective To evaluate the improvement effects of Jianpi Qingre Tongluo Fang(健脾清热通络方)on immune-inflammatory indicators in patients with ankylosing spondylitis(AS)of dampness-heat obstruction syn-drome based on a random walk model.Methods Medical records of 816 AS patients with dampness-heat obstruc-tion syndrome treated at the department of rheumatology and immunology,the first affiliated hospital of Anhui uni-versity of Chinese medicine from June 2012 to October 2020 were reviewed.Patients were divided into two groups based on treatment methods:the combination group(Jianpi Qingre Tongluo Fang+traditional Chinese herbal decoc-tion)and the herbal group(traditional Chinese herbal decoction alone).Changes in high-sensitivity C-reactive pro-tein(hs-CRP),erythrocyte sedimentation rate(ESR),complement(C)3 and C4,and immunoglobulins(Ig)A,IgG,IgM were compared between the two groups before and after treatment.Association rule analysis was employed to ex-plore the correlation between Jianpi Qingre Tongluo Fang and improvements of immune and inflammatory markers,and a random walk model was used to analyze the persistent characteristics of therapeutic effects.Results Statisti-cally significant differences were observed in hs-CRP,ESR,IgA,and C4 levels pre-and post-treatment in the com-bination group(P＜0.05),while significant differences in hs-CRP,ESR,C3,and C4 were noted in the herbal group(P＜0.05).The combination group demonstrated superior efficacy in reducing hs-CRP,ESR,IgA and C4 compared to the herbal group(P＜0.05).Association rule analysis revealed strong correlations between Jianpi Qingre Tongluo Fang combined with herbal decoction and reductions in IgM,ESR,C3,C4,and hs-CRP.The results of the random walk model revealed that both the combination group and the herbal group improved levels of C3,C4,IgA,and hs-CRP.Notably,the combination group demonstrated more significant improvements in these biomarkers(C3,C4,IgA,hs-CRP)and exhibited sustained long-range correlations.Conclusion The combined therapy with Jianpi Qingre Tongluo Fang significantly enhances the regulation effect of immune and inflammatory indicators in AS patients with dampness-heat obstruction syndrome.The random walk model confirmed sustained improvement effects on the complement system,humoral immunity,and inflammatory responses.关键词
