耳穴周期贴压治疗原发性痛经41例临床观察
Clinical study on auricular acupoint cyclic acupressure in the treatment of primary dysmenorrhea
摘要
Abstract
Objective To compare the efficacy of auricular acupoint cyclic acupressure versus fixed auricular acupoint prescriptions in treating primary dysmenorrhea and observe changes in uterine artery hemodynamics.Methods 78 patients with primary dysmenorrhea were randomly divided into a cyclic group(41 cases)and a control group(37 cases)using a random number table.The cyclic group received auricular acupoint acupressure at primary acupoints:Uterus,Shenmen,Subcortex,Endocrine,Kidney,with additional points:Liver and Sympathetic during menstruation,Spleen and Stomach during the postmenstrual phase,and Liver and Spleen during the premen-strual phase.The control group received fixed acupoints(Uterus,Shenmen,Subcortex,Endocrine,Sympathetic,Kidney,Liver).Both groups underwent unilateral auricular acupressure,alternating ears weekly,for three consecu-tive menstrual cycles.Symptoms were assessed using the visual analogue scale(VAS)and the revised Cox menstrual symptom scale(RSS-COX2).Uterine artery hemodynamics,including pulsatility index(PI),resistance index(RI),peak systolic velocity(Vmax)during the premenstrual phase,and systolic/diastolic velocity ratio(S/D),were evaluated via Doppler ultrasound.Results After three menstrual cycles,both methods significantly alleviated pain(P＜0.01),with no significant intergroup difference(P＞0.05).The cyclic group showed significant reductions in RSS-COX2 scores for dysmenorrhea-associated symptoms at the second and third menstrual cycles(P＜0.01),while the control group only showed significant reduction at the third menstrual cycle(P＜0.05),suggesting cyclic acupre-ssure accelerates symptom relief.Uterine artery hemodynamics indicators(PI,RI,Vmax,S/D)showed no significant changes within or between groups(P＞0.05).Conclusion Auricular acupoint acupressure effectively improves pain and associated symptoms of primary dysmenorrhea patient.Cyclic regimen based on menstrual phase differentiation accelerates symptom relief and is preferred for patients with prominent accompanying symptoms.关键词
