全科-专科联合模式在社区老年2型糖尿病管理中的构建与实践研究
Construction and practice research of the general-specialty joint model in the community management of elderly type 2 diabetes mellitus

Abstract
Objective To explore the intervention effect of the general-specialty joint model in the community management of elderly type 2 diabetes mellitus.Methods A total of 120 elderly patients with type 2 diabetes mellitus admitted to Dong-fang Square Community Health Service Center,Changchun Economic and Technological Development Zone from January to December 2023 were selected as the research objects.According to the random number table method,they were divided into the experimental group(60 cases)and the control group(60 cases).The experimental group received general-specialty joint model,while the control group received the traditional routine mode.Both groups were intervened for 10 months.The control rates of fasting blood glucose,2-hour postprandial blood glucose,glycosylated hemoglobin(HbA1c)and blood pressure were compared between the two groups before and after the intervention.The summary of diabetes self care activities(SDSCA)was used to evaluate the self-management of the two groups.Results After intervention,the compliance rates of fasting blood glucose,2-hour postprandial blood glucose,HbA1c,systolic blood pressure and diastolic blood pressure in the experimental group were higher than those in the control group,and the differences were statistically significant(P＜0.05).After intervention,the scores of all dimensions of SDSCA in the experimental group were higher than those in the control group,and the differences were statistically significant(P＜0.05).Conclusion The general-specialty joint model can more effectively control the blood glucose and blood pressure levels of elderly patients with type 2 diabetes mellitus in the community,and improve the self-management ability of elderly patients with type 2 diabetes mellitus,which is of significance for delaying the progress of diabetes.关键词
