Hp感染及不同毒力抗体分型在上消化道疾病中的表达差异研究
Differential expression of Hp infection and different virulence antibody typing in associated upper gastrointestinal diseases

Abstract
Objective To investigate the Helicobacter pylori(Hp)infection status in patients with different up-per gastrointestinal diseases and the differences in the expression of different Hp virulence antibody types.Methods A total of 209 patients diagnosed with chronic superficial gastritis(CSG),chronic atrophic gastritis(CAG),peptic ulcer(PU),and gastric cancer(GC)in the Department of Gastroenterology,Gansu Provincial People's Hospital between May 2023 and February 2024 were selected.Serum Hp typing was performed and correlation analysis conducted between Hp infection rate,virulence typing and factors such as age and gender.Results The detection rates of cytotoxin associated protein A(CagA),vacuolating cytotoxin A(VacA),urease A(UreA)and urease B(UreB)antibodies were 53.7%,37.8%,42.4%and 67.0%,respectively.The Hp positive detection rate was 75.1%,with statistically significant differences between the disease groups(P<0.05).Sig-nificant differences in positive rates were found between CSG and CAG,CSG and PU,CAG and PU groups(P<0.008 3).However,age and gender showed no statistically significant difference in positive detection rates(P>0.05).Additionally,the detection rates of Hp virulence typesⅠand Ⅱwere 79.6%and 20.4%,respectively.No statistically significant difference existed in the detection rates of Hp virulence types among the upper gastrointestinal disease groups,age groups or genders(P>0.05).Except for the CAG group,there were no significant difference in the detection rates of Hp virulence types by gender or age in the other groups(P>0.05).CagA and VacA,CagA and UreB,VacA and UreA,and UreA and UreB antibodies showed only moder-ate positive correlations(0.4≤|c|<0.6).The correlations between CagA and UreA,VacA and UreB antibodies were weak.Antibody expression showed statistically significant differences among the disease groups(P<0.05).The expression of VacA and UreB antibodies differed significantly between the CSG and PU groups(P<0.008 3).VacA antibody expression correlated with gender,while the expression of other antibodies showed no significant correlation with age.Conclusion In the Lanzhou region,Hp infection strains are predominantly of Type I.Hp CagA and VacA antibodies may synergistically cause gastrointestinal damage,warranting priority for Hp eradication therapy.Age and gender are relevant factors for Hp infection in CAG patients.关键词
