血清脂阿拉伯甘露聚糖抗体检测诊断肺结核的影响因素分析
Analysis of influencing factors in diagnosis of pulmonary tuberculosis by serum LAM antibody detection

Abstract
Objective To explore the efficacy and clinical influencing factors of serum anti-lipoarabinomannan(LAM)antibody detection in the diagnosis of pulmonary tuberculosis.Methods The clinical data of 185 inpa-tients diagnosed with pulmonary tuberculosis in Lanzhou Pulmonary Hospital from July 2019 to December 2023 were retrospectively collected.The patients were divided into a tuberculosis antibody positive group and tuberculosis antibody negative group,according to the LAM antibody test results.The χ2 test,Mann-Whit-ney U test and Logistic regression analysis were used to analyze the factors influencing the test results.Re-sults Of the 185 patients with pulmonary tuberculosis,91 cases(49.19%)were positive for LAM antibody and 94(50.81%)negative for LAM antibody.Multivariate Logistic regression analysis showed that patients with cough symptoms,positive sputum smear,relapsed tuberculosis and drug-resistant tuberculosis had a high posi-tive detection rate of LAM antibodies,with OR(95%CI)scores of 2.640[1.003,6.947]and 3.119[1.463,6.647],3.079[1.233,7.687],10.729[1.199,96.036].The positive detection rate of LAM antibody in patients with hematogenous disseminated pulmonary tuberculosis was low,with an OR(95%CI)of 0.235[0.078,0.707].Conclusion LAM antibody detection has a high diagnostic value for patients with cough symptoms,positive sputum smear,relapsed tuberculosis and drug-resistant tuberculosis.However,it has a relatively low sensitivity for hematogenous disseminated pulmonary tuberculosis.It is recommended to combine a detection of LAM antigen or interferon-γ release test to improve the diagnostic efficacy.关键词
