炎症性肠病患者疾病关注状况的多中心调查与分析
A multicenter survey and analysis of disease concerns in patients with inflammatory bowel disease

Abstract
Objective To confirm the reliability and validity of the Chinese rating form for inflammatory bow-el disease patient concerns(RFIPC),analyze the core concerns of Chinese inflammatory bowel disease(IBD)of patients,explore the differences in demographic characteristics and clinical variables of IBD patients in the RFIPC and assess the correlation between RFIPC and the hospital anxiety and depression scale(HADS)and the inflammatory bowel disease self-efficacy scale(IBD-SES).Methods A multicenter cross-sectional study was conducted,including information from newly diagnosed IBD patients in 17 hospitals in Gansu Province from January to December 2022.Confirmatory factor analysis(CFA),internal consistency,and test-retest reli-ability were used to assess the reliability and validity of RFIPC.Pearson correlation analysis was employed to examine the relationships between RFIPC and HADS,IBD-SES.Results A total of 166 IBD patients were in-cluded,with 134 cases of ulcerative colitis(UC)and 32 cases of Crohn's disease(CD).The CFA results of the Chinese version of RFIPC showed the root mean square error of approximation(RMSEA)to be 0.058,and the comparative fit index(CFI)to be 0.980.The total Cronbach's α coefficient for internal consistency was 0.837,and the Cronbach's α for the four dimensions 0.963,0.962,0.982,and 0.980,respectively.In the test-retest re-liability analysis,the total intraclass correlation coefficient(ICC)was 0.933,and the ICC for the four dimen-sions were 0.870,0.734,0.934,and 0.868,respectively,indicating a good reliability and validity of RFIPC.The main concerns of IBD patients were the uncertainty of the disease,drug efficacy,risk of bowel incontinence,financial burden,and pain.There were statistically significant differences in the disease impact dimension scores between smokers and non-smokers(P<0.05),and between disease activity and the total score of RFIPC(P<0.05).The total score of RFIPC was positively correlated with the total score of HADS,the anxiety score of HADS and the depression score of HADS(r>0,P<0.05),but negatively correlated with the total score of IBD-SES and the dimensions of emotional stress,disease control,and maintenance of remission(r<0,P<0.05).Conclusion The Chinese version of RFIPC has a good reliability and validity and is suitable for assess-ing the concerns and worries of Chinese IBD patients about the disease.The most concerning and worrying is-sues for IBD patients are the uncertainty of the disease,drug efficacy,risk of bowel incontinence,financial burden,and pain.Smoking may reduce the concerns of IBD patients in the disease impact dimension,while a higher disease activity is associated with more severe overall worries in IBD patients.Moreover,IBD patients with more pronounced anxiety and depression symptoms and lower self-efficacy have a higher degree of dis-ease concern and worry.关键词
