Abstract

Objective To explore the application effect of root cause analysis method in the management of medical disputes in hospitals,and to provide reference for the application of medical dispute management tools in hospitals.Methods A total of 224 cases of medical disputes occurred in Beijing Obstetrics and Gynecology Hospital,Capital Medical University,Beijing Maternal and Child Health Care Hospital from January 2023 to December 2024 were selected as the research objects,and the pre-and post-implementation grouping methods of root cause analysis were adopted,and the medical disputes occurring in January to December 2023 as the pre-implementation group,and those occurring in January to December 2024 as the Post-management implementation group.The incidence of disputes,departments where medical disputes occurred,and the reasons for their occurrence were compared and analyzed between the two groups.Results The incidence of medical disputes after implementation was lower than that before implementation,and the difference was statistically significant(P<0.05).After implementation,the incidence of medical disputes in surgical departments and non-surgical departments was lower than that before implementation,and the difference was statistically significant(P<0.05).There no statistically significant difference in the incidence of medical disputes between the medical technology department and other departments before and after implementation(P>0.05).After implementation,the incidences of medi-cal disputes caused by institutional/process deficiencies,diagnosis and treatment deficiencies,nursing quality deficiencies,and doctor-patient communication deficiencies were lower than those before implementation,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the incidence of surgical/operational defects,examination/test defects,and safety incidents before and after implementation(P>0.05).Conclusion Medical dis-putes to strengthen the management of root cause analysis method can reduce the incidence of medical disputes,can effectively promote the continuous improvement of medical quality,to protect medical safety,thus improving the doctor-patient relationship,and promote the harmonious development of society,and is worth promoting and applying.
