Relationships of IL-10 and IL-18 levels with left ventricular geometry and function in patients with primary hypertension

目的 探讨原发性高血压(PHT)患者IL-10和IL-18与左室几何构型(LVG)及左室功能的相关性.方法 选取2023年7月至2024年5月就诊于山西医科大学第一医院确诊为PHT患者224例,分为4组:正常构型(NG)组127例、向心性重构(CR)组51例、离心性肥厚(EH)组27例和向心性肥厚(CH)组19例.另选取同期本院健康体检中心招募的年龄、性别匹配,且无高血压、心血管疾病及代谢性疾病的健康者25例作为对照组.比较5组间一般资料、血清IL-10、IL-18水平及左室参数[左室舒张末期内径(LVEDD)、室间隔厚度(IVST)、左室后壁厚度(LVPWT)、左房收缩末期前后径(LAD)、左室质量指数(LVMI)、相对室壁厚度(RWT);二尖瓣口舒张早期血流峰值速度(E)、二尖瓣口舒张晚期血流峰值速度(A)、E/A、二尖瓣环舒张早期心肌运动速度(e')、E/e'、左室射血分数(LVEF)、左室整体纵向应变(GLS)].采用Pearson相关及多因素Logistic回归分析IL-10、IL-18与LVG和左室功能的关系.结果 与对照组相比,PHT组患者血清IL-10水平降低(P<0.05),IL-18水平升高(P<0.05),其中EH组和CH组变化显著(均P<0.05).Pearson相关分析显示,IL-10与LVEDD、IVST、LVPWT、A、E/e'呈负相关(P<0.05),与e'、E/A、LVEF、GLS呈正相关(P<0.05);IL-18与LVEDD、IVST、LVPWT、A、E/e'呈正相关(P<0.05),与e'、E/A、LVEF、GLS呈负相关(P<0.05).多因素Logistic回归显示,IL-10是CR、EH及CH的独立保护因素(P<0.05);IL-18 是CR、EH及CH的独立风险因素(P<0.05).结论 IL-10和IL-18与PHT患者左室结构及功能密切相关,可能参与PHT患者左室结构和功能异常的过程.

Objective To investigate the correlations between interleukin-10(IL-10),interleukin-18(IL-18),and left ventricular geo-metry(LVG),left ventricular function in patients with primary hypertension(PHT).Methods A total of 224 PHT patients in the First Hospital of Shanxi Medical University from July 2023 to May 2024 were enrolled and divided into four groups:normal geometry(NG,n=127),concentric remodeling(CR,n=51),eccentric hypertrophy(EH,n=27),and concentric hypertrophy(CH,n=19).Ad-ditionally,25 age-and sex-matched healthy individuals without hypertension,cardiovascular diseases,or metabolic disorders were re-cruited from the health examination center as control group.General clinical data,serum IL-10 and IL-18 levels,and left ventricular parameters,including left ventricular end-diastolic diameter(LVEDD),interventricular septal thickness(IVST),left ventricular pos-terior wall thickness(LVPWT),left atrial diameter(LAD),left ventricular mass index(LVMI),relative wall thickness(RWT),peak early diastolic flow velocity of the mitral valve orifice(E),peak late diastolic flow velocity of the mitral valve orifice(A),E/A ratio,peak early diastolic mitral annular velocity(e'),E/e'ratio,left ventricular ejection fraction(LVEF),and global longitudinal strain(GLS)were compared among the five groups.Pearson correlation and multivariate Logistic regression analyses were used to evaluate the associations of IL-10 and IL-18 with LVG and left ventricular function.Results Compared with control group,serum IL-10 level decreased while IL-18 level increased in PHT groups(all P<0.05),especially EH group and CH group(all P<0.05).Pearson correlation analysis revealed that IL-10 was negatively correlated with LVEDD,IVST,LVPWT,A,and E/e'(P<0.05),and positively correlated with e',E/A,LVEF,and GLS(P<0.05).Conversely,IL-18 was positively correlated with LVEDD,IVST,LVPWT,A,and E/e'(P<0.05),and negatively correlated with e',E/A,LVEF,and GLS(P<0.05).Multivariable Logistic regression analysis demonstrated that IL-10 was an independent protective factor for CR,EH and CH(P<0.05),whereas IL-18 emerged as an independent risk factor for CR,EH and CH(P<0.05).Conclusion IL-10 and IL-18 are closely related to left ventricular structure and function in PHT pa-tients,suggesting that they may be involved in the process of abnormal left ventricular structure and function in PHT.
