Abstract
Objective To observe the clinical efficacy and safety of minocycline ointment combined with tinidazole tablets in the treatment of patients with severe chronic periodontitis(CP)treated with low-intensity pulsed ultrasound subgingival scaling.Methods Patients with severe CP were divided into control group and treatment group by cohort method.Both the control group and the treatment group received low-intensity pulsed ultrasound subgingival scaling.The control group was given tinidazole tablets after scaling(1.0 g each time,once a day for 5 days),while the treatment group was treated with minocycline on the basis of the control group(an appropriate amount of minocycline ointment was taken and placed into the gingival sulcus of patients after ultrasonic supragingival scaling,and the drug was administered once).Both groups of patients were treated for 5 days.The periodontal health indexes[plaque index(PLI),probing depth(PD),clinical attachment loss(CAL)],gingival fluid inflammatory factors[transforming growth factor-β(TGF-β),interleukin-6(IL-6),IL-1β,monocyte chemotactic protein-1(MCP-1)],tooth mobility and chewing function indexes(occlusal time,left and right occlusal force balance,chewing efficiency)were compared between the two groups,and the safety evaluation was performed.Results Sixty-three cases were enrolled in the control group and 63 cases were enrolled in the treatment group.After treatment,the total effective rates of treatment group and control group after treatment were 95.24％(60 cases/63 cases)and 82.54％(52 cases/63 cases)respectively,and the difference was statistically significant(P＜0.05).After treatment,the PD values in treatment group and control group were(2.65±0.53)and(3.19±0.57)mm;the PLI scores were(1.56±0.28)and(1.72±0.31)points,the CAL scores were(2.42±0.29)and(2.69±0.37)points;the MCP-1 levels in treatment group and control group were(63.59±11.96)and(75.13±12.58)ng·L-1;the IL-6 levels were(56.92±8.05)and(60.15±7.62)pg·mL-1;the levels of TGF-β were(2.37±0.35)and(2.16±0.31)ng·mL-1,the levels of IL-1β were(33.12±6.73)and(29.64±7.13)ng·L-1;the tooth mobility values were(1.16±0.23)and(1.39±0.21)mm,the left and right occlusal force balance values were(9.34±1.82)％and(10.62±2.13)％,the occlusal time was(0.31±0.06)and(0.35±0.07)s;the chewing efficiency values were 0.49±0.10 and 0.57±0.13,respectively(all P＜0.05).The adverse drug reactions in the two groups were mainly dizziness,skin itching and gastrointestinal reactions.The total incidences of adverse drug reactions in treatment group and control group were 4.76％and 3.17％,without significant difference(P＞0.05).Conclusion Minocycline combined with tinidazole tablets in the clinical treatment of patients with severe CP receiving low-intensity pulsed ultrasound subgingival scaling can enhance the clinical efficacy and periodontal health level,relieve the local inflammatory response,reduce the tooth mobility and enhance the chewing function,without increasing the incidence of adverse drug reactions.关键词
