集采政策下人工耳蜗植入人群的临床特征与手术趋势演变:基于单中心首月百例患者的回顾性研究
Clinical Characteristics of Cochlear Implant Recipients and Trends of CI Surgery Under Centralized Procurement Policy:A Single-Center Retrospective Review of 114 Cases in the Initial Month of Policy Implementation

Abstract
Objective This study aims to analyze the clinical characteristics of cochlear implant patients,trends in surgical selection,and management of complications after implementation of the centralized procurement policy,to optimize policy implementation and clinical practice.Methods Data from 114 cochlear implant patients(138 ears)who underwent surgery from March 3 to March 31,2025 following implementation of the centralized procurement policy were retrospectively reviewed,including demographic characteristics,history of hearing loss,preoperative interventions,types of surgery and complications.Results The number of cochlear implant patients in the first month under the new policy(114,138 ears)represented a significant increase compared to previous years.Patients age ranged from 8 months to 74 years,with a marked increase of adult proportion from 23.09%to 55.26%.Atypical cases increased from 23.91%historically to 42.98%,Bilateral implants also increased from 7.00%before the new procurement policy to 21.05%.The rate of preoperative usage of hearing aids was 21.05%in children and 35.09%in adults,with the majority of Categories of Auditory Performance-Ⅱ(CAP-Ⅱ)scores between levels 0-2.The Speech Intelligibility Rating(SIR)scores was primarily at level 1(17.54%)in children,in contrast to level 5 in a significant proportion of adults(21.05%).Postoperative stress responses and complications included acute tinnitus(16.58%),dizziness(6.63%),fever(4.47%),and flap edema in one case,indicating the need for further optimization of complication management plans prior to surgery.Conclusion The centralized procurement policy significantly expanded the beneficiary population for cochlear implants,promoting surgery in adults,complex cases and bilateral implants.However,preoperative education and management of complications remain to be augmented.The information in this study provide a basis for future policy optimization and clinical practice.关键词
