EUS-FNA在胰腺占位性病变诊断中的临床意义
Clinical significance of EUS-FNA in the diagnosis of pancreatic space-occupying lesions

Abstract
Objective The purpose of this study is to determine the clinical value of endoscopic ultrasonography-guided fine-needle aspiration biopsy(EUS-FNA)in the identification of pancreatic lesions,as well as the impact of relevant multi-factors.Methods Data of 77 patients with pancreatic illness who had EUS-FNA in our hospital were collected,including complete clini-cal,puncture,and pathology information.Patients with positive EUS-FNA puncture pathology as the observation group and pa-tients with negative EUS-FNA puncture pathology as the control group.Between the two groups,variables such as age,gender,location of the mass,size,number of puncture needles,cystic solidity,biliopancreatic duct dilatation,and aberrant tumor mark-ers CEA and CA199 were compared.The sensitivity,specificity,and accuracy of EUS-FNA diagnosis were determined.The chi-square test and Fisher's exact test were used for single factor analysis of categorical data.The multivariate regression analysis of influencing factor were performed by logistic binary regression analysis.Results EUS-FNA were successfully performed in all 77 patients.The success rate of one puncture was 100%and there was 1 patient who only had abdominal pain.The accuracy of EUS-FNA needle biopsy was 76.62%,the sensitivity was 80.60%,and the specificity was 50.00%.There was no significant difference in the positive rate of EUS-FNA puncture in different parts,lesion diameter,puncture times and whether the biliopan creatic duct was dilated(P＜0.05).The positive rate of EUS-FNA puncture of solid or cystic,CEA and CA199 was statistically different(P＜0.05).On multivariate analysis,the size and whether cystic and solid of lesion,the level of CEA and the positive rate of EUS-FNA were statistically significant differences.The possibility of positive EUS-FNA puncture in cases with lesion di-ameter＞4 cm was 18.38 times that of lesions with lesion diameter≤4 cm.The possibility of positive EUS-FNA puncture in sol-id cases was 23.61 times higher than that in cystic cases.The possibility of positive EUS-FNA puncture in cases with CEA＞3.5 ng/mL is 4.95 times that in cases with CEA≤3.5 ng/mL.Conclusion EUS-FNA is a diagnostic technique with high accuracy and safety in the diagnosis of pancreatic space-occupying lesions.The size and whether cystic and solid of lesion,the level of CEA and the positive rate of EUS-FNA were statistically significant differences.关键词
