Abstract
Objective To explore and analyze the clinical effect of percutaneous unilateral puncture curved implant vertebro-plasty for the treatment of osteoporotic thoracolumbar fractures. Methods One hundred and six patients with osteoporotic thoraco-lumbar vertebral fractures who underwent percutaneous unilateral puncture and arc-shaped implantation vertebroplasty in our hos-pital from March 2019 to March 2022 were selected as the observation group, and 100 patients with osteoporotic thoracolumbar vertebral fractures who underwent conventional straight puncture and bilateral balloon expansion of the vertebral body followed by injection of bone cement were selected as the control group. The perioperative indicators of the two groups of patients were com-pared,as well as the visual analogue scale(VAS)before the surgery and 1 year after the surgery, the relative height of the af-fected vertebral body, bone density, and serum Omentin-1,ODF,and OCIF levels. Results Compared with the control group, the observation group had shorter surgical time, shorter X-ray exposure time, and more bone cement injection, with statistically significant differences(P<0.05).There was no statistically significant difference in the proportion of bone cement leakage be-tween the two groups. There was no statistically significant difference in VAS scores and relative height of lesion vertebral bodies between the two groups before the surgery,1 day after the surgery, and 3 months after the surgery.Compared with the control group, the observation group had a lower VAS score and a higher relative height of the lesion vertebral body one year after surgery, with a statistically significant difference(P<0.05).After the surgery, the lumbar spine and femoral neck bone density were higher, serum Omentin-1 was higher, and ODF and OCIF were lower in the two groups compared to before the surgery. The ob-servation group had higher lumbar spine and femoral neck bone density, higher serum Omentin-1,and lower ODF and OCIF com-pared to the control group after the surgery, and the differences were statistically significant(P<0.05).Conclusion Percuta-neous unilateral puncture with curved implantation for vertebroplasty in the treatment of osteoporotic thoracolumbar fractures has better perioperative indicators, lower pain level after 1 year of follow-up,higher relative height of the affected vertebral body, and effective improvement in bone density and serum levels of Omentin-1,ODF,and OCIF.关键词
