Abstract
Objective To explore the risk factors for postoperative delirium in elderly patients with femoral intertrochanteric fracture.Methods A retrospective analysis was performed on the clinical data of 233 elderly patients with femoral intertrochan-teric fracture admitted to the Department of Orthopedics in our hospital from August 2018 to December 2021.According to wheth-er postoperative delirium occurred,patients were divided into the delirium group and the non-delirium group.The following in-dicators were compared between the two groups:gender,age,body mass index(BMI),smoking history,drinking history,un-derlying comorbidities,Singh index,American Society of Anesthesiologists(ASA)classification,anesthesia method,operation time,interval from injury to operation(surgical timing),blood loss,blood transfusion volume,and the difference between blood loss and blood transfusion volume.Univariate analysis was used to screen potential risk factors,and then multivariate Logistic re-gression analysis was performed on the screened risk factors to identify independent risk factors for postoperative delirium.Results A total of 47 patients developed delirium,with an incidence rate of 20.17%.Univariate analysis showed statistical differences in anesthesia method,Singh index,operation time,surgical timing,and the difference between blood loss and blood transfusion volume.Multivariate Logistic regression analysis indicated that anesthesia method,Singh index,operation time,surgical timing,and the difference between blood loss and blood transfusion volume were all independent risk factors for delirium.Conclusion Anesthesia method,Singh index,operation time,surgical timing,and the difference between blood loss and blood transfusion volume are independent risk factors for postoperative delirium in elderly patients with femoral intertrochanteric fracture.The Singh index holds important clinical significance in the early prediction of delirium in such patients.关键词
