Abstract
Objective:Paclitaxel-based chemotherapeutic agents are the core drugs in cancer treatment and have been widely used in clinical practice.However,chemotherapy-induced peripheral neuropathy(CIPN)caused by paclitaxel poses a major challenge that significantly affects patients'quality of life and treatment outcomes.Cryotherapy,owing to its simplicity,safety,and non-invasive nature,has become an effective strategy for preventing CIPN.Nevertheless,no consensus has been reached regarding the optimal cryotherapy temperatures on paclitaxel-induced CIPN,providing clinical reference for appropriate temperature selection.
Methods:A stratified random grouping method was used to enroll 129 eligible cancer patients,who were randomly assigned to 3 groups:Group A(cryotherapy at 16℃),Group B(cryotherapy at 19℃),and Group C(cryotherapy at 22℃).During chemotherapy,the right hand of each patient received cryotherapy,while the left hand served as the control without intervention.CIPN was assessed after the 3rd and 6th chemotherapy cycles.Researchers evaluated skin reactions,recorded adverse events,and compared CIPN incidence between treated and untreated hands.CIPN grading followed the National Cancer Institute's Common Terminology Criteria for Adverse Events(CTCAE 5.0),and statistical methods were applied for data analysis.
Results:Of the 129 enrolled patients,105 completed the study,with 24 excluded due to incomplete chemotherapy.No patients reported intolerance to cryotherapy or other adverse events.After the 3rd chemotherapy cycle,no statistically significant difference in CIPN incidence was observed between treated and untreated hands across all groups(P>0.05).After the 6th cycle,both Group A and Group B showed significantly lower CIPN incidence in the treated hands compared to untreated hands(P<0.05),whereas Group C showed no significant difference(P>0.05).Between group comparisons revealed that the CIPN incidence in treated hands was significantly lower in Group A and Group B than in Group C(P<0.05).However,there was no significant difference between Group A and Group B(P>0.05).
Conclusion:The effectiveness of cryotherapy in preventing CIPN is closely related to both the temperature used and the chemotherapy cycle.Lower cryotherapy temperatures are key to effective prevention of CIPN.A temperature of 16℃may serve as a baseline setting,while 19℃may be suitable for patients with low cold tolerance.The efficacy of cryotherapy at 22℃requires further evaluation.关键词
