CYP3A5基因多态性联合血清miR-543对急性脑梗死患者的预后价值
Prognostic value of CYP3A5 gene polymorphism combined with serum miR-543 in patients with acute cerebral infarction

Abstract
Objective:Acute cerebral infarction progresses rapidly and often causes irreversible neurological damage,leading to poor prognosis.It is crucial to identify reliable prognostic biomarkers to investigate clinical outcomes.This study aims to evaluate the predictive value of cytochrome P450(CYP)3A5 gene polymorphism combined with serum microRNA-543(miR-543)levels for poor prognosis in acute cerebral infarction patients. Methods:A total of 176 patients diagnosed with acute cerebral infarction at the 7th People's Hospital of Zhengzhou between May 2021 and January 2024 were enrolled.Based on the modified Rankin Scale(mRS)scores at 90 days post-discharge,patients were categorized into a good prognosis group(mRS≤2,n=92)and a poor prognosis group(mRS>2,n=84).Serum miR-543 levels were measured using real-time quantitative PCR,and CYP3A5 genotypes were identified via PCR.Multivariate Logistic regression was used to identify independent risk factors for poor prognosis.Receiver operating characteristic(ROC)curves were used to assess the predictive value of the CYP3A5 rs776746 GG genotype combined with miR-543 levels. Results:Three genotypes(AA,GA,and GG)were identified at the CYP3A5 rs776746 locus.The poor prognosis group had a longer onset-to-thrombolysis time,higher serum miR-543 levels,a greater proportion of patients carrying the GG genotype or G allele,National Institutes of Health Stroke Scale(NIHSS)scores≥15,and hypertensive(all P<0.001).Both the GG genotype and elevated miR-543 were independent risk factors for poor prognosis(both P<0.05).The areas under the curve(AUCs)for predicting poor prognosis using the GG genotype,miR-543,and their combination were 0.702,0.833,and 0.932,respectively.The corresponding sensitivities were 52.38%,75.68%,and 92.86%,and specificities were 88.04%,78.33%,and 93.48%,respectively. Conclusion:The presence of the CYP3A5 rs776746 GG genotype and high serum miR-543 levels are associated with poor prognosis in acute cerebral infarction patients.Their combination significantly enhances predictive accuracy.关键词
