帕金森病认知损害的临床特点及对冻结步态的影响
Features of cognitive impairment in Parkinson's disease and its effect on the freezing of gait

Abstract
Objective To investigate the features of cognitive impairment in PD and effect of cognitive impairment on the freezing of gait(FOG)in order to provide the evidence for the management of cognitive impairment and FOG.Methods We recruited 808 cases which were diagnosed as PD,recorded cognitive status and completed MMSE scale in the Neurological Department of Xuanwu Hospital from May 26,2015 to January 8,2022.All clinical materials were collected which included H-Y stages,UPDRS-Ⅲ,MMSE,ADL scores and so on,and were analyzed.Results The whole incidence of cognitive impairment was 30.07%in hospitalized PD patients.The incidence was 27.66%in course<5 years and 40.54%in course>10 years;Compared to course<5 years,PD course>10 years can increase the 1.78-fold for the risk of cognitive impairment.Compared with age≤49 years,age>49 years had 4.04-fold increased risk of cognitive impairment(P<0.05).Cognitive impairment was significantly correlated with both ADL score and UPDRS-Ⅲ score,with correlation coefficients of 0.43 and-0.25,respectively(P<0.05),cognitive impairment increased the risk of FOG by 1.75-fold(P<0.05).In the onset of FOG,cognitive impairment has an interaction with constipation,which can increase the risk of FOG by 2.95-fold(P<0.05).Conclusion Age>49 years and disease duration>10 years are independent risk factors for cognitive impairment in PD.Cognitive impairment is moderately positively correlated with ADL and is an independent factor leading to a decline of ADL in PD.Cognitive impairment in PD can increase the risk of FOG by 1.75-fold.Cognitive impairment and constipation have an interaction in FOG,which can increase 2.95-fold risk.关键词
