牙周指标与动脉硬化性脑小血管病的相关性
Correlation between periodontal indicators and arteriosclerotic cerebral small vessel disease in patients with periodontitis

Abstract
Objective To analyze the association between periodontal indicators and cerebral small vessel disease,and investigate if moderate to severe periodontitis is an independent risk factor for CSVD.Methods The study included 91 subjects with periodontal disease who visited the First Affiliated Hospital of Zhengzhou University from January 2020 to December 2022.A retrospective analysis was conducted on the clinical data and periodontal indicators,including dental defect,probing depth,clinical attachment level,plaque index,sulcus bleeding index,gingival index and calculus index.The severity of periodontitis was graded according to the classification of periodontitis.CSVD was diagnosed by brain magnetic resonance imaging in the past 3 months,and its severity was graded based on the total burden score of CSVD.The general information and periodontal indicators were compared between patients with or without CSVD,between patients with mild and moderate to severe CSVD.Binary Logistic regression analysis was further applied to determine whether periodontitis is independently correlated with the occurrence of CSVD and the progression from mild to moderate-severe CSVD.Results Ninety-one patients were included in this study.Multivariate regression analysis found that age(OR=3.189,95%CI:1.092-9.308,P=0.034),primary hypertension(OR=6.190,95%CI:2.658-14.416,P<0.001),and moderate to severe periodontitis(OR=1.021,95%CI:1.004-1.039,P=0.017)were independent risk factors for CSVD,and brushing frequency≥2 per day(OR=0.366,95%CI:0.140-0.959,P=0.041)was protective factor for CSVD.Age(OR=3.190,95%CI:1.150-8.846,P=0.026),primary hypertension(OR=5.995,95%CI:2.461-14.603,P<0.001),and moderate to severe periodontitis(OR=1.029,95%CI:1.002-1.058,P=0.038)were independent risk factors for mild CSVD progressing to moderate to severe CSVD.Conclusion Periodontitis is independently associated with the occurrence of CSVD and the progression of mild CSVD to moderate-severe CSVD.Active treatment to improve patient periodontal indicators may be one of the effective measures to prevent the occurrence of CSVD and the progression of mild CSVD to moderate-severe CSVD.关键词
