帕金森病患者血清sIL-2R TLR4 IGF-1及Hcy水平与病情严重程度的关系
Relationships among serum sIL-2R,TLR4,IGF-1,Hcy levels and disease severity in Parkinson's disease patients

Abstract
Objective To investigate the relationships among serum soluble interleukin 2 receptor(sIL-2R),Toll-like receptor 4(TLR4),insulin-like growth factor-1(IGF-1),and homocysteine(Hcy)levels and disease severity in patients with Parkinson's disease(PD).Methods The clinical data of 116 PD patients(PD group)admitted to the First Hospital of Qinhuangdao from 2022 to 2024 were retrospectively analyzed(39 cases in the early group,50 cases in the middle group,and 27 cases in the late group).In addition,82 healthy subjects who underwent physical examination in our hospital during the same period were selected as control group.The levels of serum sIL-2R,TLR4,IGF-1 and Hcy were compared between the two groups and the patients with different diseases.Spearman correlation analysis was used to analyze the correlations of sIL-2R,TLR4,IGF-1,Hcy and Hoehn-Yahr(H-Y)classification.The predictive value of sIL-2R,TLR4,IGF-1 and Hcy on the severity of PD was analyzed by receiver operating characteristics(ROC)curve.Results The levels of sIL-2R,TLR4,and Hcy in the PD group were higher than those in the control group,and the level of IGF-1 was lower than that in the control group(P<0.05).Comparison of serum sIL-2R,TLR4,and Hcy levels in PD patients with different conditions:early group<middle group<late group,and comparison of IGF-1 levels:early group>middle group>late group(P<0.05).The correlation analysis showed that sIL-2R,TLR4,Hcy were positively correlated with H-Y grade,and IGF-1 was negatively correlated with H-Y grade(P<0.05).The ROC curve analysis showed that area under the curve(AUC)of sIL-2R in predicting PD disease was 0.819(95%CI:0.744-0.814,P<0.001),AUC predicted by TLR4 was 0.786(95%CI:0.690-0.881,P<0.001),AUC predicted by IGF-1 was 0.842(95%CI:0.819-0.961,P<0.001),AUC predicted by Hcy was 0.791(95%CI:0.655-0.907,P<0.001),AUC of the combined of four(0.919,95%CI:0.866-0.972,P<0.001)was significantly higher than that of the single index.Conclusion PD patients have high expressions of sIL-2R,TLR4,and Hcy,and low expression of IGF-1,and the above factors are closely associated with the severity of PD.The combined detection of sIL-2R,TLR4,IGF-1,and Hcy has a certain value in predicting the condition of PD patients.关键词
