2014至2023年浦东新区异位妊娠危重孕产妇发生情况及影响因素
The influencing factors of critical maternal conditions due to ectopic pregnancy in Pud-ong New Area from 2014 to 2023

Abstract
Objective:To analyze the incidence and trends of critical maternal condi-tions due to ectopic pregnancy in Pudong New Area of Shanghai from 2014 to 2023,to explore possible influencing factors,and to provide a basis for formulating effective preventive meas-ures.Methods:A retrospective analysis was conducted on 195 cases of critical maternal condi-tions due to ectopic pregnancy in Pudong New Area from 2014 to 2023.Additionally,390 cases of ectopic pregnancy that were not critical during the same period were selected as controls to analyze the influencing factors of critical conditions in ectopic pregnancy.Results:The inci-dence of critical maternal conditions showed a decreasing trend from 2014 to 2023(IRR=0.9398,P<0.001),however,there was no significant decline in the incidence of critical mater-nal conditions due to ectopic pregnancy(IRR=0.9790,P=0.416).For 51.8%of critical ec-topic pregnancy cases,the first-visit department was non-obstetrics and gynecology(such as e-mergency rooms,internal medicine,and surgery).Case-control analysis revealed statistically significant differences in days of amenorrhea,days of vaginal bleeding,shock,nausea and vomi-ting,first-visit department,and visit time between the critical and non-critical groups(P<0.05).Multivariable logistic regression analysis showed that a longer duration of amenorrhea was a risk factor for critical conditions in ectopic pregnancy(P<0.05).Conclusion:Strengthe-ning science popularization and education among women of reproductive age,fostering good menstrual recording habits,and seeking medical attention promptly upon discovering any abnor-malities.Healthcare professionals should thoroughly inquire about menstrual cessation to facili-tate accurate triage and early identification of ectopic pregnancy.Implement closed-loop man-agement for suspected or confirmed ectopic pregnancies to effectively prevent them from critical conditions.关键词
