前列腺癌根治术联合比卡鲁胺对前列腺癌患者尿动力学、肿瘤标志物和免疫功能的影响
Effects of radical prostatectomy combined with bicalutamide on urodynamics,tumor markers,and immune function in patients with prostate cancer

Abstract
Objective To investigate the efficacy of radical prostatectomy combined with bicalutamide on prostate cancer patients.Methods A total of 80 prostate cancer patients admitted to No.904 Hospital from May 2019 to May 2023 were enrolled and randomly divided into control group(n=40)and observation group(n=40)using the odd-even number method.Radical prostatectomy alone was applied in the control group,while patients in the observation group received radical prostatectomy combined with bicalutamide(50 mg,oral administration,once a day for 18 months).Clinical indicators before and after treatment were compared between the two groups,including International Prostate Symptom Score(IPSS),Quality of Life(QoL)score,urodynamic parameters,tumor markers level,immune function indicators,postoperative complications,adverse reactions,and recurrence rate.Results After treatment,the observation group showed significantly lower IPSS scores and higher QoL scores compared to the control group(P＜0.05).Residual urine volume was lower,but average urine flow rate(Qave)and maximum urine flow rate(Qmax)were higher compared to the control group(P＜0.05).The levels of serum prostate-specific antigen(PSA),free prostate-specific antigen(fPSA),and vascular endothelial growth factor(VEGF)were significantly lower in the observation group(P＜0.05).The level of CD3+,CD4+,and CD4+/CD8+levels were higher,while CD8+level was lower compared to the control group(P＜0.05).The main postoperative complications in the two groups were delayed lymphatic leakage,urinary tract infection,urinary incontinence,and anastomotic leakage.There was no significant difference in the incidence of postoperative complications between the two groups(P=0.069).The main adverse reactions included sexual dysfunction,diarrhea,fatigue,and hot flashes.There was no significant difference in the incidence of adverse reactions between the two groups(P=0.381).All patients were followed up for 18 months.The recurrence rate was 25.0%in the control group and 5.0%in the observation group(P=0.028).Conclusions Radical prostatectomy combined with bicalutamide can effectively improve clinical symptoms,urodynamic parameters,tumor marker levels,and immune function in prostate cancer patients,reduce recurrence,and demonstrate good safety.关键词
