基于长期护理保险需求的老年人机构照护意愿及其影响因素研究
The willingness of elderly people to receive institutional care based on long-term care insurance demand and its influencing factors

Abstract
Objective To analyze the influencing factors of elderly people's willingness to receive institutional care,provide scientific evidence and suggestions for the development of formal care in institutions for further meeting their care needs.Methods Based on the assessment data of the long-term care insurance in the information system from January to May 2023,the binary logistic regression model was employed to analyze the influencing factors of the institutional care needs of the elderly.Then,according to the current living situations of the research subjects,they were divided into the home-based group and the institutionalized group.Stratified analysis was conducted to explore the factors influencing the institutional care needs of these two groups,and the factors'differences were compared.Results Gender,age,education level,family support level,elderly-oriented living environment,and health status are significantly correlated with their institutional care needs(P＜0.05).The stratified analysis reveals that the elderly aged 80 or above are more prone to maintain their current care patterns,either home-based or in insitutions.The elderly with inadequate family support are more likely to switch to institutional care(OR=5.00).In terms of the elderly-oriented environment,the installation of handrails in rooms and bathrooms of elderly care institutions is a promoting factor for them to stay in institutional care(OR:1.11 and 3.04,respectively),while the installation of handrails in toilets shows an opposite effect(odds ratio=0.75).In addition,home-based elderly people with cognitive impairment(OR=2.01)and poor compatibility with others(OR=2.08)showe a demand for institutional care,but the needs might not have been met for those living in the institutions.Conclusions It is important to pay attention to family support and reduce the burden of caregivers for elderly people at home by offering domestic services.The elderly-oriented decoration should consider the actual disability situation of elderly people.Lastly,attention needs to be paid to dementia care in the future through improving professionalism and other aspects.关键词
