The resources allocation and services for osteoporosis prevention and treatment in Shanghai communities

Abstract
Objective To evaluate the osteoporosis prevention and treatment of Community Health Service Centers(CHSCs)in Shanghai,and offer suggestions for future work.Method In March 2024,a census method was adopted to systematically investigate osteoporosis prevention and treatment among all CHSCs in Shanghai,covering four dimensions:the allocation of health personnel,related resources,health services,and two-way referral systems.Results A total of 247 CHSCs from 16 districts in Shanghai were surveyed.Among them,51 centers(20.6%)has established osteoporosis outpatient clinics.A total of 1496 clinicians,134 public health physicians,and 795 registered nurses has received training on osteoporosis.In terms of equipment allocation,105 centers(42.5%)are equipped with ultrasonic bone densitometers,64(25.9%)with dual-energy X-ray absorptiometry(DXA)scanners,and 22(8.9%)with bone density therapeutic devices.For medication availability,209 centers(84.6%)have bone resorption inhibitors,226(91.5%)with bone mineralization promoters,and 14(5.7%)with bone formation promoters.In the constext of health education,157 CHSCs(63.6%)have carried out osteoporosis health education,mainly by means of offline health seminars and workshops(55.1%),distribution of brochures(30.8%),and one-on-one health consultation and guidance(22.7%).Additionally,43 centers(17.4%)have carried out osteoporosis high-risk population screening.Furthermore,189 centers(76.5%)have implemented a two-way referral system,with a total of 14,500 referred cases.In 2023,the number of referral cases included 9,975 upward referrals and 4,495 downward ones.Conclusions CHSCs in Shanghai have established the basic practices for osteoporosis diagnosis and treatment.The pilot program for osteoporosis prevention and treatment practices has achieved impressive results and expanded to non-pilot centers.The next step is to further improve the operational mechanism of osteoporosis prevention and treatment in these centers.关键词
