缺血性脑白质脱髓鞘病变的中医证候与临床特点的相关性研究
Correlation study between TCM patterns and clinical characteristics of ischemic cerebral white matter demyelinating lesions

Abstract
Objective This study aimed to explore the prevalence and clinical features of Traditional Chinese Medicine(TCM)patterns among patients suffering from ischemic cerebral white matter demyelination(ICWMDL).Methods Clinical data and TCM diagnostic information were collected from 88 ICWMDL patients at the Department of Encephalopathy,Dongfang Hospital,Beijing University of Chinese Medicine,between September 1,2020,and January 31,2022.Analysis focused on the correlation between ICWMDL-related TCM patterns and comorbidities,family/personal history,imaging findings,and scores on mobility,cognitive,and emotional scales.Results Hypertension was the predominant comorbidity in these patients,with dyslipidemia and diabetes mellitus also frequently observed.Correlation analysis between TCM pattern and clinical characteristics demonstrated the following statistically significant associations:yang deficiency positively correlated with dyslipidemia,blood deficiency with diabetes mellitus,and phlegm-turbidity with alcohol consumption history(all P<0.05).Blood deficiency showed negative correlation with Short Physical Performance Battery(SPPB)scores,qi deficiency with Montreal Cognitive Assessment(MoCA)scores,while blood stasis positively correlated with Hamilton Depression Rating Scale(HAMD)scores,and marrow depletion syndrome with Hamilton Anxiety Scale(HAMA)scores(all P<0.05).Conclusions ICWMDL predominantly affects middle-aged and older adults,with key pathologies involving phlegm obstruction and marrow deficiency,alongside blood deficiency complicated by stasis.There is a significant correlation between TCM pattern elements and clinical symptoms.关键词
