Abstract
Objective:To investigate whether early implementation of one-lung ventilation in thoracoscopic surgery can accele-rate lung collapse,and to evaluate the effects of continuous endotracheal negative pressure suction on the speed of lung collapse and postoperative complications.Methods:A total of 75 patients undergoing thoracoscopic radical resection of lung cancer from Oct.2021 to Jul.2022 were selected.Patients were randomly divided into three groups using a random number table,with 25 patients in each group.Group A received early implementation of one-lung ventilation,Group B received continuous endotracheal negative pressure suction,and Group C received conventional management.The lung collapse scores at 5 minutes(T1),10 minutes(T2),and 20 minutes(T3)after pleural opening were compared among the three groups,as well as the time required to achieve satisfactory lung collapse.The alveola-rarterial oxygen difference and oxygenation index at 10 minutes(T4)and 30 minutes(T5)after one-lung ventilation were also com-pared.Postoperative complications were evaluated among the three groups.Results:A statistically significant difference was observed in lung collapse scores among the three groups(P<0.05).The lung collapse scores in Group B and Group C were higher than those in Group A.The time required to achieve satisfactory lung collapse was shorter in Group B and Group C compared to Group A,and the time in Group B was shorter than that in Group C(P<0.05).At T4,the alveolar-arterial oxygen difference in Group A was higher than that in Group B,and higher in Group C than in Group B.The oxygenation index in Group B was higher than that in Group A(P<0.05).However,at T5,there were no statistically significant differences in the alveolar-arterial oxygen difference and oxygenation index among the three groups.No statistically significant differences were found in the incidence of intraoperative and postoperative complica-tions among the three groups.Conclusions:Early implementation of one-lung ventilation delays the process of lung collapse,while con-tinuous endotracheal negative pressure suction is a safe and effective method to promote lung collapse.关键词
