机器人与腹腔镜辅助全胃切除术治疗局部进展期胃癌的近期疗效分析
Short-term efficacy analysis of robotic versus laparoscopic-assisted total gastrectomy for locally advanced gastric cancer

Abstract
Objective:To compare the short-term therapeutic effect between robotic and laparoscopic-assisted radical total gas-trectomy with lymph node dissection.Methods:Clinical data of 125 patients who underwent radical total gastrectomy from Jan.2021 to Dec.2022 were retrospectively analyzed.The robotic group(n=63)received robot-assisted surgery,while the laparoscopic group(n=62)underwent laparoscopic surgery.The general characteristics,intraoperative findings,postoperative conditions,and complications of the two groups were compared and analyzed.Results:There was no significant difference in baseline characteristics between the two groups(P＞0.05).The robotic group showed less intraoperative blood loss[(117.62±51.04)mL vs.(162.90±71.13)mL,t=4.095,P＜0.001],more second-station lymph nodes harvested[(11.71±3.71)vs.(9.92±3.41),t=2.818.P=0.006],and higher total lymph nodes yield[(39.22±9.60)vs.(35.45±11.05),t=2.037,0.044].No differences were found in operative time,first-station lymph node count,number of positive lymph nodes,pathological T stage,and pathological N stage between the two groups(P＞0.05).The robotic group had shorter postoperative analgesia duration[(1.97±0.80)d vs.(2.39±0.75)d,t=3.005,P=0.003],earlier first postoperative exhaust[(3.62±0.92)d vs.(4.77±1.00)d,t=6.716,P＜0.001],and shorter postoperative hospital stay[(9.35±1.48)d vs.(10.98±2.08)d,t=5.059,P＜0.001].Time to ambulation,oral liquid diet intake,and drainage tube removal showed no differences between the two groups(P＞0.05).The overall postoperative complication rates(25.8％vs.19.0％,P=0.365)and 30-day readmission rates due to complications(1.6％vs.1.6％,P=0.991)were comparable between laparoscopic group and robotic group.Conclusions:Robot-assisted radical total gastrectomy is safe and feasible,demonstrating advantages in lymph node dissection,in-traoperative bleeding control,and postoperative recovery,and is worthy of clinical promotion and application.关键词
