带蒂大网膜包裹术在腹腔镜直肠癌根治术中的应用研究
Application study of pedicled greater omentum wrapping in laparoscopic radical resection of rectal cancer

Abstract
Objective:To investigate the efficacy of pedicled greater omentum wrapping in laparoscopic radical resection for rectal cancer.Methods:Clinical data of 87 patients who underwent laparoscopic radical resection for rectal cancer(Dixon procedure)from Jan.2020 to Jun.2024 were retrospectively analyzed.The wrapping group(n=42)received pedicled omental wrapping of the an-astomosis,while the non-wrapping group(n=45)did not.The incidence and grade of anastomotic leakage,operation time,intra-abdom-inal infection,intraoperative blood loss,total incision length,severity of low anterior resection syndrome(LARS)one month postopera-tively,postoperative hospital stay,time to first ambulation,and time to first anal exhaust were compared between the two groups.Results:The incidence and the grade of anastomotic leakage in the wrapping group were significantly lower than those in the non-wra-pping group(P＜0.05).The wrapping group had earlier time to first ambulation,shorter time to first anal exhaust,shorter postoperative hospital stay,lower rate of intra-abdominal infection,and less LARS score one month postoperatively compared to the non-wrapping group(P＜0.05).There were no significant differences in operation time,intraoperative blood loss,and total incision length between the two groups(P＞0.05).Conclusions:The application of pedicled greater omentum wrapping of the anastomosis in laparoscopic radical surgery for rectal cancer is safe and effective,can reduce the incidence and grade of anastomotic leakage,decrease intra-abdominal in-fection,alleviate the severity of LARS one month postoperatively,and shorten hospital stay,providing clinical benefits for patients.关键词
