Abstract
Objective:To explore the influence of modified vesicourethral anastomosis technique in laparoscopic radical prosta-tectomy on perioperative indexes and urinary continence function.Methods:A retrospective analysis was conducted on the clinical data of patients who underwent laparoscopic radical prostatectomy with modified sandwich vesicourethral anastomosis technique(group A,n=98)and those who underwent laparoscopic radical prostatectomy with modified double-handed vesicourethral anastomosis technique(group B,n=103)from Jun.2022 to Jun.2023.Perioperative indexes(surgical time,intraoperative blood loss,urinary catheter removal time,total hospital stay),urinary continence function(urinary continence recovery rate,1-hour pad test),tumor related indexes(trans-forming growth factor-β1,transforming growth factor-β3,prostate specific antigen)and postoperative complications were compared between the two groups,and the tumor-free survival in the two groups was analyzed by Kaplan-Meier curve.Results:The operation time of group B was significantly shorter than that of group A(P＜0.05),but there was no significant difference in intraoperative blood loss,time of catheter removal and total hospital stay between the two groups(P＞0.05).The international consultation on incontinence ques-tionnaire-short form scores of the two groups at 3 and 6 months after operation were lower than those at 1 month after operation,the urine volume tested by 1-hour urine pad was reduced compared with that at 1 month after operation,but there was no significant difference in the above indexes at each time point between the two groups(P＞0.05).At 1 and 3 months after operation,the levels of transforming growth factor-β1,transforming growth factor-β3 and prostate specific antigen in the two groups were significantly lower than those before operation(P＜0.05),but there was no significant difference between the two groups(P＞0.05).There was no significant difference in the complications between the two groups(P＞0.05).During the 1-year postoperative follow-up,there was no significant difference in tumor-free survival time between Group A and Group B[(11.70±1.66)months vs.(11.46±1.6)months,log-rank x2=0.190,P=0.663].Conclusions:The modified two-handed vesicourethral anastomosis technique reduces operative time without compromising postoperative recovery,controlling urinary function,safety of surgery,or long-term oncological outcomes compared to the modified sand-wich vesicourethral anastomosis technique.关键词
