心脏超声参数与血清GDF11、IRAK-1水平联合评估冠心病患者冠状动脉狭窄程度的价值研究
Value of Cardiac Ultrasound Parameters Combined with Serum GDF11 and IRAK-1 Levels in the Evaluation of Coronary Artery Stenosis in Patients with Coronary Heart Disease

Abstract
Objective:To investigate the value of cardiac ultrasound parameters combined with serum growth differentiation factor 11(GDF11)and interleukin-1 receptor-associated kinase-1(IRAK-1)levels in evaluating the degree of coronary artery stenosis in patients with coronary heart disease(CHD).Methods:The data of 134 patients with CHD admitted to our hospital from September 2021 to September 2023 were retrospectively analyzed as CHD group(42 cases in mild group,56 cases in moderate group and 36 cases in severe group according to Gensini score)and 134 healthy cases as control group.The filling speed(β value)and the maximum number of microvesicles(A value)were measured by cardiac ultrasound.Serum GDF11 and IRAK-1 levels were determined by ELISA.Spearman correlation analysis and ROC curve analysis were performed.Results:β value(P<0.05)and A value(P<0.05)in CHD group were significantly lower than those in control group,while the GDF11 level decreased(P<0.05)and the IRAK-1 level increased(P<0.05).With the increase of stenosis degree,β value and A value decreased,GDF11 decreased,IRAK-1 and Gensini scores increased(all P<0.05).Spearman correlation analysis showed that Gensini score in CHD patients was negatively correlated with β value(rS=-0.456,P<0.05),A value(rS=-0.501,P<0.05),GDF11(rS=-0.426,P<0.05),and IRAK-1 level(rS=0.523,P<0.05),and positively correlated with IRAK-1 level(rS=0.523,P<0.05).The AUC of severe stenosis detected by cardiac ultrasound(β value,A value)combined with serum GDF11 and IRAK-1 was 0.948(95%CI:0.896~0.979),the sensitivity was 89.9%,and the specificity was 82.7%,which was significantly higher than that by single detection(P<0.05).Conclusion:Cardiac ultrasound combined with serum GDF11 and IRAK-1 can effectively evaluate the degree of coronary artery stenosis in patients with CHD,especially in the diagnosis of severe stenosis.关键词
