能谱CT成像在急性胰腺炎初步诊断及病情评估中的应用研究
Application of Spectral CT Imaging in the Initial Diagnosis and Disease Assess-ment of Acute Pancreatitis

Abstract
Objective:To investigate the application value of Spectral CT imaging in the initial diagnosis and disease assessment of acute pancreatitis.Methods:78 patients with acute pancreatitis treated in our hospital from March 2020 to December 2023 were selected as the observation group,and 64 healthy volunteers who underwent physical examination during the same period were selected as the control group.All patients underwent energy spectrum CT examination,and the CT value and iodine concentration of the two groups were measured to explore the diagnostic value of spectrum CT imaging in acute pancreatitis.Then,according to the Ranson score,the patients in the observation group were divided into three subgroups:mild group,moderate group and severe group.The CT values and iodine concentration in portal venous phase and arterial phase of different stages of acute pancreatitis were recorded to further explore the value of Spectral CT imaging in the evaluation of acute pancreatitis.Results:The CT value at different energy levels(40 keV to 140 keV),iodine concentration and NIC values in arterial phase and portal venous phase in the observation group were significantly lower than those in the control group(P<0.05).With the progression of the disease,the CT values at different energy levels(40 keV to 140 keV)of Spectral CT imaging in the arterial phase and portal venous phase of patients with acute pancreatitis decreased significantly,and the iodine concentration and NIC values in the arterial phase and portal venous phase also decreased significantly(P<0.05).The ROC curve showed that the AUC of iodine concentration and NIC values in the diagnosis of acute pancreatitis were 0.933 and 0.936,respectively.The AUC of iodine concentration value and NIC values in the diagnosis of acute pancreatitis grading was 0.987 and 0.958.Conclusion:Spectral CT imaging has a certain value in the diagnosis and evaluation of acute pancreatitis,and can provide accurate guidance for clinical diagnosis and treatment.关键词
