老年人群高尿酸血症及痛风发病影响因素研究进展OA
Research advances in the influencing factors of hyperuricemia and gout in the elderly population
随着人口老龄化加剧,老年人群高尿酸血症(Hyperuricemia,HUA)及痛风患病率显著上升,已成为重要的公共卫生问题.本文综述了老年人群HUA及痛风发病的多种影响因素及其交互作用机制.发现老年人群HUA及痛风的发病与遗传易感性、代谢紊乱(如胰岛素抵抗、脂质异常)、生活方式(高嘌呤饮食、乙醇摄入、久坐)、共病状态(高血压、慢性肾病、肥胖)及药物使用(利尿剂、免疫抑制剂)等多因素密切相关.其中,与年龄相关的肾功能退化及尿酸排泄能力下降是老年人群HUA高发主要因素.近年来研究表明,环境因素(季节变化、空气污染)与遗传因素(ABCG2、SLC2A9基因多态性)的交互作用进一步增加老年人群的患病风险.针对老年患者的特殊性,需采取分层管理策略,结合精准医学理念优化生活方式干预、药物选择及共病管理.未来研究应深入探讨衰老相关分子机制、新型生物标志物及靶向治疗策略,为老年人群HUA及痛风的个体化防治提供依据.
With the accelerating pace of population aging,the prevalence of hyperuricemia(HUA)and gout in elderly populations has risen significantly,emerging as a critical public health concern.This article provides a systematic review of the multiple influencing factors and their interactive mechanisms in the pathogenesis of HUA and gout among the elderly.The development of HUA and gout in the elderly populations is closely linked to genetic predisposition,metabolic abnormalities(such as insulin resistance and dyslipidemia),lifestyle(high-purine diet,alcohol intake,physical inactivity),comorbid conditions(hypertension,chronic kidney disease,obesity),and medication use(diuretics,immunosuppressants).Among these,age-related renal dysfunction and reduced uric acid excretion capacity constitute the core pathophysiological basis for the high prevalence of HUA in older individuals.Recent studies highlight that the interaction between environmental factors(seasonal variations,air pollution)and genetic factors(polymorphisms in ABCG2 and SLC2A9 genes)further amplifies disease risk in the elderly.Considering the unique characteristics of elderly patients,a stratified management strategy is advocated,integrating the principles of precision medicine to optimize lifestyle modifications,pharmacotherapy selection,and comorbidity management.Future research should delve into aging-associated molecular mechanisms,novel biomarkers,and targeted therapeutic strategies to provide references for personalized prevention and treatment of HUA and gout in elderly population.
